Tecniche di Breath - Hold a confronto nel trattamento del carcinoma mammario

The dose delivered to the left anterior descending artery (LAD) in radiotherapy of left-sided breast cancer increased risk of myocardial Infarction. In particular, it has been shown that twenty years after radiotherapy to the left breast cancer there is a double probability to infarction compared with patients treated with radiation therapy who have tumors in right breast.All of which has led with the use of multiple irradiation techniques to minimize the dose to anterior descending artery, such as the deep inspiration breath-hold technique (DIBH) that can decrease radiation dose delivered to the heart. The study was performed on a TrueBeam and ClinaciX Varian linear accelerators, which is equipped with Varian Realtime Position Management (RPM) system, and BrainLAB ExacTrac gating systems. In this study, the two systems were assessed on accuracy of both motion tracking and radiation delivery control. The two techniques allow to quantify and compensate the respiratory movement to analyse the organ motion problems.In addition, we compared times of patient positioning, setting and treatment delivery by the verification of the treatment fields position on different days to document. We analysed advantages and criticalities of both systems trying to explain how these variables can be affect by delivery and workflow.This study involved 16 female patients diagnosed with left sided breast cancer under 65 years’ patients treated by adjuvant radiation therapy; the first 8 treated with the Breath Hold Tracking technique and the next 8 with Breath Hold Gaiting. Patients were followed up during planning and delivery treatment.From the comparison, positive and negative sides emerge. We present an extensive comparison between the use of RPM and Tracking systems on their technical capabilities and operating approach. Infrared tracking system continuously monitors patient positioning throughout treatment that allows for optimal patient positioning in all six degrees of freedom enabling an improved setup accuracy compared to translational corrections alone. In addition, daily patient setup and delivery treatment was lowest than RPM system. In addition, systematic and random errors was lowest than RPM system, even if only little. On the other and, RPM gating system provides both respiratory gating for respiration-synchronized imaging and the treatment beam is turned on, and RPM automatically gates the beam on and off instantly according to the selected upper and lower gating thresholds.The Gating technique, performed through the RPM system, has the advantage of delivering the radiant dose to the patient only when it is able to maintain the breath within the pre-set band. The main disadvantage is that this technique requires a greater active collaboration on the part of the patient.The possibility of having both methods guarantees a greater number of patients the opportunity to perform breath hold therapy, order safeguard possible future radio-induced cardiac pathologies.Nella radioterapia del carcinoma mammario sinistro la dose erogata all’arteria discendente anteriore (DA) aumenta il rischio di sviluppare un infarto del miocardio. In particolare, in queste pazienti è stato dimostrato che, circa venti anni dopo il trattamento, la probabilità di avere un infarto raddoppia rispetto a quelle sottoposte a terapia radiante sulla mammella destra.Tutto ciò ha portato all’impiego di molteplici tecniche di irradiazione che hanno reso possibile la riduzione della dose all’arteria discendente anteriore, mediante l’esecuzione della terapia in inspirazione forzata (Breath Hold). Scopo del nostro studio è quello di mettere a confronto due tecniche di Breath Hold in uso nel nostro istituto: la Breath Hold Gating, attuata con il sistema dedicato denominato Real-time Position Management, RPM (Varian Medical Systems, Palo Alto, USA), e la Breath Hold Tracking, espletata tramite il software ExacTrac della BrainLab (BrainLab AG, FeldKirchen; Germania). Le due tecniche permettono di quantificare e compensare il movimento respiratorio in modo da analizzare con maggiore attenzione le problematiche derivanti dall’organ-motion intra-frazione. Sono stati confrontati i tempi di posizionamento, di impostazione e di trattamento, e le tolleranze di errore nella valutazione del set-up, mettendo in evidenza i vantaggi e le criticità di entrambe le tecniche, e valutandone l’influenza sia sulla delivery che sul workflow.Nello studio sono state arruolate 16 pazienti, di età inferiore a 65 anni, affette da carcinoma mammario sinistro, eleggibili di terapia radiante adiuvante; 8 di queste sono state trattate con tecnica Breath Hold Tracking e 8 con Breath Hold Gating. Le pazienti sono state seguite durante la fase di pianificazione e di trattamento.Da un’attenta analisi delle due tecniche a confronto sono emersi i vantaggi e gli svantaggi di ognuna di esse. Il Tracking respiratorio si è rivelato un sistema che permette di valutare in real-time la posizione della paziente, attraverso sei differenti coordinate spaziali e la possibilità di un auto-centraggio permesso dall’integrazione del sistema ExacTrac e del letto BrainLab sul TrueBeam. Nella fase di simulazione il tempo macchina è minore rispetto alla tecnica Gating. Nella fase di trattamento, l’elevato tempo di set-up è compensato dal minor tempo richiesto per la terapia. Ulteriori vantaggi, anche se minimi, sono stati evidenziati nella valutazione di errori sistematici ed errori random. Lo svantaggio principale della tecnica di Tracking, è riconducibile al fatto che la macchina permette l’erogazione della dose anche al di fuori del range di tolleranza.La tecnica Gating, espletata attraverso il sistema RPM, ha il vantaggio di erogare la dose alla paziente solo quando il respiro viene mantenuto all’interno della banda preimpostata. Lo svantaggio principale, è rappresentato dai tempi di acquisizione TC e di erogazione della terapia che risultano essere più elevati rispetto alla tecnica Tracking, e da una maggiore collaborazione attiva da parte della paziente.L’impiego di entrambe le metodiche radioterapiche permette ad un maggior numero di pazienti di eseguire la terapia con tecnica breath hold, con una riduzione della possibile tossicità cardiaca radio-indotta
