PETA BAKTERI DAN POLA RESISTENSI ANTIMIKROBA PADA ANAK DENGAN KEGANASAN DI RSUD DR SOETOMO SURABAYA PERIODE JANUARI-DESEMBER 2015

Children with malignancy often have an infection that is a cause of increased morbidity and mortality rates so that antimicrobials should be immediately given. Unwise use of antimicrobials will lead to antimicrobial resistance and changes in bacterial patterns. Objective: Identify the microbiological spectrum and patterns of antimicrobial resistance in children with malignancy suspected of having bacterial infection in Dr. Soetomo, Surabaya Metode: Observational cross sectional. This study was conducted on all medical records of children with malignancy suspected of having infection (fever clinic), culture examination and results of which are treated in the pediatric ward Oncology-Hematologi Dr. Soetomo hospital in Surabaya from January 2015 to December 2015. Result: Of the 299 children, there were 272 children who fulfilled the inclusion criteria with 836 cultures, derived from blood specimens (57.9%), urine (31.2%), sputum (0.8%), swabs and pus (7, 9%), feces (1.6%), LCS (0.1%) and pleural fluid (0.5%). The mean age of this study subjects was 6.2 years with the lowest age of 4 months and the highest 17 years. The most age group who experienced malignancy was age 1-5 years, which was 51.8%. Boys suffer more ferocity than girls with a 2: 1 ratio. The most common malignancies were hematologic malignancy (66.5%), with most comorbidities being neutropenia (46.5%). Results of sterile cultures were found in 69.4% of specimens. Positive culture results were found in 30.6% of specimens. The more common bacteria found was gramnegative bacteria of 67.2% compared to gram-positive bacteria. The most common gram-negative bacteria found were Escherichia coli (30%), Pseudomonas aeruginosa (22.4%) and Klebsiella pneumonia and ESBL bacteria found 27%, MRSA 2/15 (13.3%) specimens. All gram-negative and gram-positive bacteria have been resistant to ampisillin
