The paper is trying to answer the question which word-formative types, of the ones capable of being isolated by structural analysis of derivative, transformed into word-formation models of surnames. The conducted analyses revealed that word-formation models of surnames were created only from the types that became obsolete. The up-to-date types, in turn, did not form a separate model. The created models were used for structural modification of the already existing surnames, the formation of new ones and Polonization of foreign surnames
