Effectiveness and safety of intravenous zoledronic acid for the treatment of postmenopausal osteoporosis: a systematic review

Introduction: The osteoporosis is a skeletal disorder characterized by the compromising of the bone strength, that predisposes to an increase in the risk of fractures; it is, therefore, an important cause of morbidity, mortality and loss of the life quality among postmenopausal. The bisphosphonates, agents that inhibit the bone reabsorption mediated by the osteoclasts, constitute, at the present time, the first-line treatment for the postmenopausal osteoporosis. They have been demonstrating reduction in the risk of vertebral and non vertebral fractures in postmenopausal women , but the low adhesion to the treatment is still a challenge that commits the effectiveness of this intervention. The use of an intermittent regimen of the zoledronic acid, a new and potent bisphosphonate , administered intravenously in an annual infusion, seems to constitute a promising alternative to the daily or weekly oral administration of the others bisphosphonates. However, a systematic revision of the literature is obligatory to define how effective and safe is the zoledronic acid for the treatment of the osteoporosis in postmenopausal women. Objective: to determine the effectiveness and safety of the zoledronic acid in the treatment of the postmenopausal osteoporosis. Methods: search strategy - a) online: PUBMED, BASE, LILACS, CENTRAL (COCHRANE LIBRARY); b) hand search :list of references. Selection criteria - a)types of studies: all of the controlled and randomized studies eligible; b): types of participants: only trials with postmenopausal women will be include; d)types of interventions: trials with zoledronic acid versus placebo or active comparator; d) types of outcome measures: percent changes in bone mineral density and biochemical bone markers; number of fractures, including vertebral and non vertebral fractures; toxicity; e) data collection and analyses: three reviewers will inspect, in an independent way, the references found by the search strategy and they will apply the inclusion criteria in the selected studies, using the criteria of methodological quality described in Cochrane Handbook.Introdução: A osteoporose é uma desordem esquelética caracterizada pelo comprometimento da força óssea, que predispõe a um aumento no risco de fraturas; é, portanto, uma causa importante de morbidade, mortalidade e prejuízo da qualidade de vida entre as mulheres na pós-menopausa. Os bisfosfonatos, agentes que inibem a reabsorção óssea mediada pelos osteoclastos, constituem, na atualidade, o tratamento de escolha para a osteoporose pós-menopausal. Eles têm demonstrado capacidade de redução no risco de fraturas vertebrais e nãovertebrais em mulheres na menopausa, mas a baixa adesão ao tratamento ainda é um desafio que compromete a eficácia desta intervenção.O uso de um regime intermitente do ácido zoledrônico, um novo e potente bisfosfonato administrado intravenosamente em uma infusão anual, parece constituir uma promissora alternativa à administração oral diária ou semanal dos demais bisfosfonatos. Entretanto, uma revisão sistemática da literatura é a obrigatória para se definir o quão efetivo e seguro é o ácido zoledrônico para o tratamento da osteoporose pós-menopausal. Objetivos: determinar a efetividade e segurança do ácido zoledrônico no tratamento da osteoporose pós-menopausal. Métodos: estratégia de busca a) online: PUBMED, EMBASE, LILACS, CENTRAL (COCHRANE LIBRARY), b) manual:lista de referências. Critérios de seleção- a)tipos de estudos: todos os estudos controlados e randomizados elegíveis; b)tipos de participantes: apenas ensaios clínicos randomizados com mulheres pósmenopausadas serão incluídas; c) tipos de intervenção:todos os ensaios com ácido zoledrônico versus placebo ou comparador ativo; d) tipos de avaliações:alterações percentuais nas medidas da densidade mineral óssea e marcadores bioquímicos ósseos, número de fraturas, incluindo fraturas vertebrais e não-vertebrais, toxicidade; e) coleta de dados e análises: três revisores inspecionarão, de maneira independente, as referências encontradas pela estratégia de busca e aplicarão os critérios de inclusão nos estudos selecionados, usando os critérios de qualidade metodológica descritos no Cochrane HandbookTEDEBV UNIFESP: Teses e dissertaçõe
