CD4+ cells and HIV-1 viral load dynamics during and after antiretroviral prophylaxis for HIV-1 mother-tochild transmission

Purpose: To assess the impact of HIV-1 mother-to-child transmission antiretroviral prophylaxis cessation after delivery on CD4+ cells and HIV-1 viral load dynamics in HIV-1 infected women. Methods: The study subjects came from a retrolactive cohort of HIV-1 infected pregnant women that received medical care at the NUPAIG – Hospital São Paulo between 2000 and 2005. The ones who had CD4+ cell count over 300 cells/mm³; received prophylaxis with high-activity antiretroviral therapy (HAART); stopped antiretroviral within four weeks after delivery; and had laboratorial assessments before, during and after prophylaxis have been included in the analysis. Results: Seventy-five pregnancies, including three women with two gestations, were assessed. Twenty-four cases had received antiretroviral treatment before prophylaxis. Median baseline CD4+ cell count was 573 cells/mm³. Prophylaxis started after 26.6 weeks of gestation and lasted up to 11.7 weeks in 75% of the cases. Prophylactic scheme was changed in twelve pregnancies. Protease inhibitor was included in 33 prophylaxes. The median CD4+ cell count increase during prophylaxis was 25% in relation to the baseline value. For 75% of the cases, prophylaxis resulted in undetectable HIV-1 viral load within less than 6.7 weeks. HIV-1 viral load was not undetected during prophylaxis in five cases. Time to achieve undetectable HIV-1 viral load was reduced if baseline HIV-1 viral load was below 100,000 copies/ml and antiretroviral scheme was not changed during prophylaxis. After prophylaxis cessation, CD4+ cell count did not significantly fall and HIV-1 viral load plateau was close to the preprophylaxis level. The estimated average time for CD4+ cell count falls below 300 cell/mm³ was 3,5 years for 120,6 person-year of study follow-up. The post-prophylaxis period with CD4+ cell count over 300 cells/mm³ was positively influenced by the baseline CD4+ cell count and the CD4+ increase during prophylaxis, and negatively affected by the use of antiretroviral drugs before prophylaxis and the fail to undetect HIV-1 viral load during prophylaxis. Conclusions: CD4+ cells and HIV-1 viral load dynamics during and after antiretroviral prophylaxis may be favored by choosing a potent and well-tolerated prophylactic antiretroviral scheme. Women that received such prophylaxis seem to delay the CD4 cell count fall to those levels that are indication of treatment.Objetivo: Avaliar o impacto da estratégia de interrupção de uso de profilaxia anti-retroviral para transmissão materno-infantil de HIV-1 após o parto na dinâmica de células CD4+ e na carga viral de HIV-1 nas mulheres infectadas por HIV-1. Métodos: Estudo de coorte observacional retrolactiva que incluiu todas as gestantes infectadas por HIV-1 atendidas no NUPAIG –Hospital São Paulo entre os anos 2000 a 2005 que tiveram valor de células CD4+ antes da profilaxia superior a 300 células/ml; receberam profilaxia com tratamento antiretroviral de grande atividade (HAART); interromperam o uso de medicações anti-retrovirais em até quatro semanas após o parto e tiveram avaliações laboratoriais antes, durante e pós a profilaxia. Resultados: Setenta e cinco gestações, incluindo três mulheres com duas gravidezes, foram avaliadas. Vinte-e-quatro casos tinham antecedente de uso de antiretrovirais antes da profilaxia. A mediana de CD4+ basal foi de 573 células/mm³. A profilaxia foi iniciada após 26,6 semanas de idade gestacional e durou até 11,7 semanas em 75% dos casos. Esquema profilático foi trocado em doze gestações. Inibidor de protease fez parte de 33 profilaxias. A mediana do incremento no valor de células CD4+ durante a profilaxia em relação ao valor basal foi 25%. A profilaxia levou a indetecção de carga viral de HIV-1 em até 6,7 semanas em 75% dos casos, sendo que somente em cinco gestações não foi atingido esse objetivo. O tempo para indetectar a carga viral de HIV-1 foi reduzido quando a carga viral inicial de HIV-1 foi menor de 100.000 cópias/ml e foi usado único esquema profilático. Após a interrupção da profilaxia, não houve queda significativa do valor de CD4+ e a carga viral de HIV-1 retornou ao patamar pré-profilaxia. O tempo médio estimado para ter valor de CD4+ inferior a 300 células/mm³ após profilaxia foi de 3,5 anos e foi calculado com seguimento de 120,6 pessoas-ano. O período pós-profilaxia com nível de CD4+ acima de 300 células por mm³, no modelo de riscos proporcionais de Cox, foi influenciado positivamente pelo nível de CD4+ pré-profilaxia e pelo ganho de CD4+ durante a profilaxia e negativamente pela exposição a anti-retrovirais antes da profilaxia e pela carga viral de HIV-1 detectável no fim da profilaxia. Conclusões: A dinâmica de células CD4+ e de carga viral de HIV-1 durante e após a profilaxia anti-retroviral pode ser influenciada positivamente pela escolha de esquema antiretroviral profilático potente e bem tolerado. A mulher que recebe dita profilaxia parece postergar a queda de células CD4+ a níveis que indiquem a necessidade de tratamento.TEDEBV UNIFESP: Teses e dissertaçõe
