Follow-up of endovascular treatment of direct carotid-cavernous fistulas

Purpose: The objective of the present study was to evaluate the long-term clinical and radiological progression of patients treated with detachable balloons. Methods: The present study was developed from January 2006 to March 2007, from the evaluation of 13 patients submitted for endovascular treatment for direct CCF at the Hospital das Clínicas/Incor and the Hospital São Paulo (UNIFESP), in São Paulo, SP, Brazil, from October 1997 to February 2006. Results: We found 8 cases of traumatic fistulas and 5 cases of spontaneaus fistulas. The ICA was preserved in 11 patients (84,6%). The therapeutic occlusion was done in 2 cases (15,4%). The follow-up was variable between 19 e 128 months (mean 66,2 months). The ophtalmologics exams were abnormals in 8 patients (61,5%). The magnetic ressonance image (MRI) and the magnetic ressonance angiography (MRA) of the skull was proceded in all patients. We found 11 pseudoaneurysms in 10 of the 11 patients in who the ICA was preserved. The digitil subtraction angiography (DSA) was performed in 5 of these patients and we note no differences relateds to the morphology, the size, the neck or the number of pseudoaneurysm that had been showed by MRI and MRA. Conclusion: The endovascular treatment of DCCF with detachable balloons shown to be efficient and stable at long-term follow-up. We found pseudoaneurysms in 91% of the cases that the ICA was preserved, all of them assymptomatics. The MRI and MRA showed corresponding accuracy with the DSA in the diagnostic of intracarvenous pseudoaneurysms.Objetivo: Avaliar, nos pacientes com fístulas carotidocavernosas diretas (FCCD), a eficácia e a estabilidade tardia do tratamento endovascular com balões destacáveis; avaliar a ocorrência de pseudoaneurismas nos pacientes cuja artéria carótida interna (ACI) foi preservada, bem como a sua correlação clinicorradiológica; avaliar a acurácia da ressonância magnética (RM) e da angiorressonância magnética (ARM) no diagnóstico por imagem dos pseudoaneurismas. Métodos: Neste trabalho, realizamos os controles clínicos e radiológicos de 13 pacientes que foram submetidos ao tratamento endovascular de FCCD, entre outubro de 1997 e fevereiro de 2006. Resultados: 8 pacientes apresentavam fístulas traumáticas e 5 mostravam fístulas espontâneas. A ACI foi preservada em 11 (84,6%) pacientes. A oclusão terapêutica da ACI foi realizada em 2 (15,4%) casos. O tempo de realização do controle variou de 19 a 128 meses (média de 66,2 meses). O exame oftalmológico revelou alterações em 8 (61,5%) pacientes, presentes desde a data do tratamento e sem sinais de evolução. A RM e a ARM do crânio foram realizadas em todos os pacientes e evidenciaram 11 pseudoaneurismas em 10 (90,9%) dos pacientes cuja ACI foi preservada. Cinco desses pacientes foram submetidos à angiografia digital cerebral (ADC) que caracterizou os pseudoaneurismas observados à ARM, não havendo diferenças significativas quanto à forma, à dimensão, ao colo ou ao número. Conclusão: O tratamento endovascular de FCCD com balões destácaveis mostrou-se eficaz e estável a longo prazo. Encontramos pseudoaneurismas em 91% dos casos com preservação da ACI, sendo todos assintomáticos. A RM e a ARM demonstraram acurácia semelhante à da ADC no diagnóstico dos pseudoaneurismas do segmento cavernoso da ACI.TEDEBV UNIFESP: Teses e dissertaçõe
