Transcutaneous bilirubin in fully breastfed late preterm, early term, and full-term newborn infants in the first week of life

Background. Since most of newborn infants are fully breastfed in the first week of life, it is important to understand the behavior of physiological bilirubin values to guide mothers during hospital stay and follow them up after hospital discharge. Objective: To determine the increase and decrease of transcutaneous bilirubin (TcB) in late-preterm infants, early term and term newborns fully breastfed in the first week of life. Method: A prospective cohort study with 210 healthy, fully breastfed neonates born from March/09-March10, with the following criteria: 35-39 weeks of gestation, birth weight >2000g, Apgar 5 min >7, without any congenital malformation or hemolytic disease. All were at rooming-in before 6 hours of life. Each newborn had sternum TcB (JM 103 Minolta) and body weight measured at 24, 48, 96 , 144 and 192 hours of life. The TcB was considered until initiation of phototherapy, according to AAP 2004. TcB mean estimates were compared by ANOVA repeated measures between late preterm (n=60), early term (n=60), and full term (n=90) newborn infants. Maternal and neonatal characteristics were compared between groups by one-way ANOVA, Kruskal Wallis, X2, and Fisher test (significant p2000g, Apgar >7 no 5º minuto, sem malformação congênita/doença hemolítica Rh, em alojamento conjunto antes de 6h de vida e aleitamento materno exclusivo até o 8º dia. Na 24ª e 48ª h de vida durante a internação e na 96ª, 144ª e 192ªh no domicílio, a Btc (JM 103 Minolta) foi mensurada no esterno e o peso foi determinado em balança digital. A BTc foi considerada até a indicação de fototerapia. As estimativas médias de BTc foram comparadas por ANOVA com medidas repetidas entre PT tardios (PTT) de 35-36 semanas (n=60), termo precoce (TP) de 37 e 38 semanas (n=60) e de termo (T) de 39 a 41 semanas (n=90). Características maternas e neonatais foram comparadas com one-way ANOVA, Kruskal Wallis, qui-quadrado e teste exato de Fisher, considerando-se significante p<0,05. Resultados. Os 3 grupos foram semelhantes quanto à idade materna 23+7 anos, cor parda/negra 85%, primiparidade 54%, hipertensão/diabetes 12%, parto vaginal 70%, ocitocina 64%, peso adequado/IG 83%, sexo masculino 49%, incompatibilidade ABO 10%, mamada na 1ª hora 57% e perda de peso na 48ªh de vida 5,9+2,0% (máximo 11,1%), mas foram diferentes quanto ao uso de fototerapia (PTT-22%; TP-5%; T-1%; p<0,001). A BTc apresentou evolução diferente nas três faixas de IG (p<0,001) e ao longo do tempo (p<0,001). A estimativa média da BTc nos grupos PTT, TP e T foi, respectivamente, com 24 horas de vida - 4,5; 3,9 e 3,1 mg/dL (p=0,037), 48 horas - 7,4; 6,3 e 4,7 mg/dL (p<0,001), 96 horas - 7,8; 7,0 e 4,4 mg/dL (p<0,001), 144 horas - 6,2; 5,8 e 3,2 mg/dL (p<0,001) e 192 horas - 4,9; 4,9; e 2,5 mg/dL (p
