PURPOSE: Perform a program evaluation of the paramedicine transitional care program, the Community Integrated Healthcare Program, to determine its effectiveness in reducing heart failure 30-day readmissions compared to usual care. BACKGROUND: Heart failure is the leading cause of 30-day readmissions in the country with a rate of approximately 21.9%. Heart failure affects 6.2 million Americans and the incidence is expected to grow to 8 million by 2030. Additionally, the cost of treating heart failure is 30.7 billion dollars annually and is anticipated to double by 2030.
