Prévalence des tendinopathies calcanéennes et patellaires de l’enfant en population générale

Introduction: Several studies have demonstrated the existence of tendinopathies in young high-level athletes. The most described tendinopathies are patellar and calcaneal tendinopathies. However, there are few documents studying these pathologies in the general population. The aim of the study presented here was to determine the prevalence of patellar and calcaneal tendinopathies in children in the general population, to determine the associations between tendinopathies and apophysitis, and to try to determine the risk factors for these pathologies. Material and Methods: The prevalence of patellar and calcaneal tendinopathies was studied in children with pain in the front face of the knee and / or the posterior face of the ankle. This population was formed retrospectively from a consultation center over 3 years. Diagnostic criteria of tendinopathy and apophysitis was clinical. Anthropological data, medical background, practised physical activities and carried out imageries were collected and described. Results: The study included 193 patients aged from 7 to 18 years old with a median of 13.7, 60% of boys and 40% of girls. 69.4% of the children had at least one tendinopathy. More specifically, 66.4% of patellar tendinopathies were found, of which 41.2% were associated with apophysosis (Osgood Schlatter and / or Sinding Larsen), and 70.3% of calcaneal tendinopathies of which 62.5% were associated with Sever's disease (p <0.05). According to the logistic regression model, practice of a jumping sport and male sex were associated with the occurrence of patellar tendinopathy (p <0.05). Conclusion: This princeps study allows to demonstrate the existence of a high prevalence of calcaneal and patellar tendinopathies in children in the general population. About half of the tendinopathies were associated with the corresponding apophysosis. In the general population, therefore, it seems necessary to integrate the diagnosis of tendinopathy into current practice in order to set up appropriate management as early as possible.Introduction : Plusieurs études ont démontré l'existence des tendinopathies chez les jeunes sportifs de haut niveau. Les tendinopathies les plus décrites sont les tendinopathies patellaires et calcanéennes. Cependant, rares sont les documents étudiant ces pathologies en population générale. Le but de l'étude ici présente était de déterminer la prévalence des tendinopathies patellaires et calcanéennes chez les enfants en population générale, de déterminer les associations entre tendinopathies et apophysoses, et d'essayer de déterminer les facteurs de risque de ces pathologies. Matériel et Méthodes : La prévalence des tendinopathies patellaire et calcanéenne a été étudiée chez des enfants présentant des douleurs à la face antérieure du genou et/ou de la face postérieure de cheville. Cette population a été constituée de manière rétrospective à partir d'un centre de consultation sur 3 années. Les diagnostics de tendinopathie et d'apophysose étaient cliniques. Les données anthropologiques, les antécédents, les activités physiques pratiquées et les imageries effectuées ont été recueillis et décrits. Résultats : L'étude comptabilisait 193 patients âgés de 7 à 18 ans pour une médiane à 13,7 ans avec 60% de garçons et 40% de filles. 69,4% des enfants avaient au moins une tendinopathie. Plus précisément, nous retrouvions 66,4% de tendinopathies patellaires dont 41,2% étaient associées à une apophysose (Osgood Schlatter et/ou Sinding Larsen) et 70,3% de tendinopathies calcanéennes dont 62,5% étaient associées à une maladie de Sever (p<0.05).
