Betel leaf has been widely used for various diseases such as toothache, mouth ulcer, oral cavity abscess, as an antiseptic, fungicide even as bactericidal. The infection of microorganisms is the presence of the growth and development of microorganisms in the body of living creatures and can cause discomfort and even disease to death in living creatures the content of antibacterial and antifungi in green betel leaf extract can be used as an anti-infective agent. so that the Creams and ointments are suitable preparations for topical treatment because they are easy to clean, the way they work in the local tissue, and not sticky.reatment is more practical, the extract of betel leaves is made in cream preparations and ointments. This type of research is non-experimental research conducted in a laboratory with maceration method for extraction. The yield obtained from maceration extraction was 13.34 grams. Organoleptic from ointment and cream preparations was brick red with a distinctive aroma of betel leaf, the pH test on cream and ointment preparations was 4 acidic, while homogeneity test was not homogeneous. while cream preparations have good homogeneity stability, testing the spreadability of ointment preparations have a good dispersion while cream has a smaller spread than ointment, for specific gravitytest both the ointment and cream preparations have aspecific gravity that is 1gr / ml.   Keywords : extraction, Creams, maceration, Methanol, ointments, piperis bettle &nbsp
