Tėkmės citometrijos analizę šlapimo pūslės vėžio

At present, bladder tumors are diagnosed and followed-up using a combination of cystoscopic inspection, cytology and histology. This is not only expensive, but also highly subjective investigation and gives less information about the underlying molecular characteristics of the tumor. In the present study, we evaluated patients with T3 and T4 bladder carcinoma (BCa) after radical cystectomy with the main focus on the prognostic importance of DNA ploidy, immunostaining of intracellular markers (CK18 and vimentin) and cell surface markers (CD31, CD45 and CD326) using flow cytometry (FCM). We have modified the original enzymatic and mechanical dissociation technique to create a single-cell suspension of bladder tissue for FCM and optimized its quality control. DNA content was determined by flow cytometry of Hoechst33342 stained nuclei. All samples were compared to internal standard of chicken erythrocyte nuclei (CEN) as an internal reference. Analysis of DNA distribution revealed the presence of normal cells in G1, S and G2M phase of cells cycle (control diploid cells) together with rare aneuploid populations of BCa cells. The robust coefficient of variation (rCV) of cells in G1 peak was above 3.0% in most of the cases and DNA Index (DI, ratio to CEN standard) between 2.0 - 3.0. The determination of DNA content coupled with the study of intracellular intermediate filaments (cytokeratin18 and vimentin) and cell lineage surface markers could be useful in providing valuable information on the clinical management of BCa patients. The above-mentioned methods were applied to 8 bladder carcinoma patient samples. Using cell surface staining of isolated live cells we were able to identify individual populations of epithelial (CD326+CD31-CD45-), endothelial (CD326-CD31+CD45-), and hematopoietic cell lineages (CD326-CD31-CD45+) using selected markers. We were more concern about cancer stem cells (CSC) and circulating tumor cells 5 (CTC). CSCs and CTCs are a subset of tumor cells that have the ability to self-renew and to generate all of the heterogeneous cells that contain a tumor and they are needed to be eliminated to provide long-term disease-free survival. In summary, our results show that multiparametric FCM provides a rapid and convenient way of screening and classification of bladder carcinoma patients. Clinical data will be released over the scope of this thesis and are not part of my thesis.Šiuo metu šlapimo pūslės augliai diagnozuojami ir toliau tiriami naudojant cistoskopinę apžiūrą bei citologinius, histologinius tyrimus. Tai yra ne tik brangus būdas, tačiau ir subjektyvus, suteikiantis ribotą kiekį informacijos apie auglio kompleksiškensę molekulinę charakteristiką. Šiame tyrime buvo tiriami pacientai su T3 ir T4 pūslės karcinoma (BCa) po radikalios cistektomijos, daugiausia dėmesio skiriant prognozių svarbai įvertinant tarpląstelinių žymenų (citokeratino 18 ir vimentino) imunodažymą, DNR ploidiją ir ląstelės paviršiaus žymenis (CD31, CD45, CD326), naudojant srovės citometriją (FCM). Naudota katalitinė ir mechaninė disociacijos technika vienos šlapimo pūslės audinio ląstelės suspensijai FCM gauti. DNR kiekis buvo nustatytas naudojant Hoechst33342 dažyto branduolio srovės citometriją. Visi mėginiai buvo palyginti su vidiniu viščiuko eritrocito branduolio (CEN) etalonu kaip kontrole. DNR pasiskirrstymo analizė parodė, kad yra normalių ląstelių G1, S ir G2M ląstelės ciklo fazėse (kontrolinės diploidinės ląstelės) bei retų aneuloidinių BCa ląstelių. Ląstelių stabilaus nuokrypio koeficientas (rCV) G1 fazėje viršijo 3.0% beveik visuose atvejuose, o DNR indeksas (DI, santykio su CEN standartas) – 2.0-3.0. DNR kiekio nustatymas kartu su epitelio (citokeratinas18 ir vimentinas) ir ląstelės paviršiaus žymenimis gali būti naudingas suteikiant vertingą informaciją apie BCa pacientų klinikinį gydymą. Aukščiau išvardinti metodai buvo pritaikyti 8 šlapimo pūslės karcinomoms. Naudojant išskirtų gyvų ląstelių paviršių dažymą galėjome identifikuoti epitelių (CD326+CD31-CD45-), endotelių (CD326-CD31+CD45-) individualias populiacijas ir hematopoetinių ląstelių linijas (CD326-CD31-CD45+) 7 panaudodami pasirinktus žymenis. Mes buvome didesnį susirūpinimą vėžio kamieninių ląstelių (CSC) ir cirkuliuojančių vėžinių ląstelių (BTK). CSCS ir CTCs yra naviko ląstelių, kurios turi galimybę savarankiškai atnaujinti ir generuoti visus nevienalyčių elementų, kuriuose yra auglys ir jų reikia pašalinti teikti ilgalaikę ligos reiškinių išgyventas poaibis. Išvadose tyrimo rezultatai rodo, jog multiparametrinė FCM suteikia greitą ir patogų būdą ekranuoti ir klasifikuoti šlapimo pūslės karcinomos pacientus.
