ENUCLEAÇÃO DE CISTO PERIAPICAL E PREENCHIMENTO COM LUMINA BONE E L-PRF: RELATO DE CASO

Introduction: The root cyst originates from a periapical granuloma with preexisting epithelium, whichconstitutes a focus of granulation tissue, chronically inflamed, at the apex of a tooth without vitality. Theprevalence of the radicular cyst corresponds to about 60% of the maxillary and mandible cysts, representingthe most common odontogenic cysts. Objective: The objective of this study is to report a clinical case ofperiapical cyst enucleation and cavity filling with biomaterial and platelet and leukocyte-rich fibrin (L-PRF).Case report: CAFS, male patient, 62 years old, asymptomatic and referred for endodontic treatment to thetooth 34. After radiological examination, radiolucent lesion was detected with defined limits involving the rootapices of the teeth 33, 34 and the region of 35 (absent), with a periapical cyst hypothesis. It was planned forthe enucleation of the cystic lesion and filling of the bone antrum with the synthetic bovine bone (LuminaBone) and L-PRF, for better healing and aid in the new bone formation, better final results of the surgery,besides reducing inflammatory reaction and also assisting in the repair of cut. Final Considerations: L-PRFpresents itself as a viable alternative in the control of inflammatory conditions, helping the gingival repairprocess and stabilizing the grafting material, thus allowing the future rehabilitation of the patient throughosseointegrable implants. A prevalência do cisto periapical corresponde a cerca de 60% dos cistos em maxila e mandíbula,representando o mais comum dos cistos odontogênicos. Objetivo: O objetivo deste trabalho é relatar umcaso clínico de enucleação de cisto periapical e preenchimento da cavidade com biomaterial e Fibrina ricaem plaquetas e leucócitos (L-PRF). Relato de caso: Paciente CAFS, gênero masculino, 62 anos,assintomático e encaminhado para tratamento endodôntico do dente 34. Após exame radiográfico foidetectado lesão radiolúcida de limites definidos envolvendo os ápices radiculares dos dentes 33, 34 e aregião do 35 (ausente), com hipótese de cisto periapical. Foi planejada a enucleação da lesão cística epreenchimento da loja óssea com o osso bovino sintético (Lumina-Bone) e L-PRF, para melhor cicatrizaçãoe auxílio na neoformação óssea, melhores resultados finais da cirurgia, além de diminuir reação inflamatóriae também auxiliar no reparo de feridas. Considerações Finais: O L-PRF apresentou-se como umaalternativa viável no controle do quadro inflamatório, auxiliando o processo de reparo gengival eestabilização do material de enxertia, permitindo assim a futura reabilitação do paciente por meio deimplantes osseointegráveis.
