Estrategia de intervención para mejorar la calidad de vida de cuidadores principales de pacientes en hemodiálisis

Introduction: chronic kidney disease causes changes that affect the life of the patient and the family system. The hemodialysis treatment and the associated complications make the patient require systematic attention not only from the health team, but also from the main caregiver, a situation that with time causes a deterioration of the caregiver's health that affects his or her quality of life.Objective: to design an intervention strategy to improve the quality of life of the main caregivers of hemodialysis patients.Methods: a qualitative-quantitative intervention study was carried out; descriptive, longitudinal and prospective, in the Hemodialysis Service of the “Arnaldo Milian Castro” Hospital in Villa Clara, in the triennium 2016-2019. The sample was made up of 63 caregivers who met the selection criteria.Results: most of the main caregivers were women from 40 to 59 years old, spouses, with an employment relationship. A moderate level of self-transcendence predominated in 65.08% of the caregivers, and among the dimensions most affected by their quality of life are emotions and behavior (24, 64.8%), followed by changes in physical appearance (21, 56.6%), some have affected rest, sleep and social life (15, 40.5%).Conclusions: the integration of Pamela Reed's theory and Betty Neuman's systems model constituted a useful theoretical support that allowed designing a nursing intervention strategy aimed at improving the quality of life and satisfaction of the main caregivers of hemodialysis patients.Introducción: la enfermedad renal crónica provoca cambios que afectan la vida del paciente y del sistema familiar. El tratamiento de hemodiálisis y las complicaciones asociadas hacen que el paciente requiera de atención sistemática no solo del equipo de salud, sino del cuidador principal, situación que con el tiempo provoca un deterioro de la salud del cuidador que repercute en su calidad de vida.Objetivo: diseñar una estrategia de intervención para mejorar la calidad de vida de los cuidadores principales de pacientes en hemodiálisis.Métodos: se realizó un estudio de intervención cualitativo-cuantitativo; descriptivo, longitudinal y prospectivo, en el Servicio de Hemodiálisis del Hospital “Arnaldo Milián Castro” de Villa Clara, en el trienio 2016-2019. La muestra estuvo conformada por 63 cuidadores que cumplieron con los criterios de selección.Resultados: la mayoría de los cuidadores principales eran mujeres de 40 a 59 años, cónyuges, con vínculo laboral. Predominó un nivel moderado de autotrascendencia en el 65,08% de los cuidadores y entre las dimensiones más afectadas de su calidad de vida se muestran las emociones y la conducta (24, 64,8%), seguidas de los cambios en la apariencia física (21, 56,6%), algunos tienen afectados el descanso, el sueño y la vida social(15, 40,5%).Conclusiones: la integración de la teoría de Pamela Reed y el modelo de los sistemas de Betty Neuman constituyeron un sustento teórico útil que permitió diseñar una estrategia de intervención de enfermería dirigida a mejorar la calidad de vida y la satisfacción de los cuidadores principales de pacientes en hemodiálisis
