The results of the fine-needle aspiration cytology in comparison with the clinical judgment and the final histological diagnosis of parotid masses

In einer retrospektiven Studie in der HNO-Klinik Würzburg wurden im Zeitraum von Januar 1997 bis Dezember 2001 394 Patienten, die an der Gl. parotis operiert wurden, untersucht. Die untersuchten Parameter waren die Ergebnisse der klinischen Beurteilung, der Fazialisuntersuchung und der Feinnadelaspirationszytologie im Vergleich mit dem endgültigen histopathologischen Befund hinsichtlich ihrer Dignität. Bei dem Vergleich der Verdachtsdiagnose der klinischen Beurteilung mit der Histologie ergab sich wie erwartet eine Sensitivität von nur 25%, da ein maligner Tumor nur selten schon durch die alleinige klinische Untersuchung erkannt wird. Bei dem Vergleich der Fazialisuntersuchung mit der Histologie wurde eine Fazialisparese als sicheres Anzeichen eines malignen Tumors gewertet. Die Sensitvität lag bei nur 6%. Bei dem Vergleich der Feinnadelpunktion mit der Histologie ergab sich eine Sensitivität von 73%. Die Feinnadelpunktion hatte die höchste Sensitivität und konnte bei Speicheldrüsenschwellungen nicht-neoplastische von neoplastischen unterscheiden.Between January 1997 and December 2001, 394 patients refered for operative therapy of the salivary gland neoplasms underwent a retrospective study at the Department of Otorhinolaryngology-Head and Neck Surgery at the University of Würzburg. The study compared result of the clinical judgment, the facial nerve examination and the fine-needle aspiration cytologie with the final histological diagnosis. The sensitivity of the clinical judgment in comparison with the histological diagnosis was 25%. As expected it is difficult to diagnose a maligne tumor only by the clinical judgment. In this study the facial paralysis was a safe indication for a maligne tumor. So, the sensitivity of the facial examination in comparison with the histological diagnosis was 6%. The sensitivity of the fine-needle aspiration cytology in comparison with the histological diagnosis was 73%. The fine-needle aspiration cytology has the highest sensitivity and is able to distinguish amoung parotid gland neoplasms
