Healthcare error is a persistent challenge for clinicians, administrators, regulators, and policy makers. Researchers argue that the number of errors originally cited by Institute of Medicine’s (IOM) landmark report, To Err is Human: Building a Safer Health System (1999) were grossly underestimated and that despite concerted efforts aimed to mitigate error in healthcare settings, error remains a persistent and difficult problem to combat.
