Analysis of Surgical Treatment of Malignant Adrenal Tumors

The aim of the study: To evaluate results of the surgical treatment of malignant adrenal tumors during the period of 1998 and 2018 in the Department of Surgery in LUHSH KC. Objectives: 1. To identify the main demographic data of the patients (gender, age, the number of operations during the period); 2. To evaluate clinical data of the operated patients; 3. To perform the analysis of the results of the surgical treatment and to evaluate the patients survival. Methodology: Retrospective clinical study analyzing patients, who had malignant adrenal tumors surgery during the period of 1998 and 2018 year in the Department of Surgery in LUHSH KC, clinical data. The study participants: Patients with malignant adrenal tumors operated between 1998 and 2018 year in the Department of Surgery in LUHSH KC. Results: The study group consisted of 62 patients, including 40 patients with primary malignant adrenal tumors and 22 metastatic adrenal tumors. The mean age of women (54,5) and the mean age of men (56,7) did not differ significantly. We found that the majority of patients in the preoperative period were bothered by low back pain (71 %) and a number of non-specific symptoms (general weakness, fever, weight loss). As many as 38 (61,3 %) patients reported more than one symptom. An increase in cortisol levels, a decrease in adrenocorticotropic hormone in the blood were found in patients with adrenal cortical cancer. In patients with malignant pheochromocytoma – an increase in catecholamines. Based on radiological examinations, 43 >5 cm tumors were identified and more often operated on in the open method, 19 <5 m tumors were more often operated on laparoscopically. The most common pathohistological diagnose in primary adrenal tumors was adrenal cortical cancer, which was more often removed during open surgery, followed by metastatic adrenal cancer – also more often removed during open surgery. The best survival was in patients with a
