The Link Between Chronic Kidney Disease Progression Risk and Clinical Conditions Most Frequently Encountered in Family Physician’s Practice

Aim of the study: To investigate chronic kidney disease (CKD) progression risk links with clinical conditions frequently encountered in family physician’s practice. Objectives: To investigate the link between CKD and arterial hypertension, diabetes mellitus (DM), obesity, metabolic syndrome and the risk of cardiovascular disease (CVD). Material and methods: Data were collected as the part of EUROASPIRE V survey. Adults without a documented CVD who had been prescribed antihypertensive medicines and/or lipid-lowering medicines and/or treatment for diabetes were eligible for the survey. Data were collected through the review of medical records, patients’ interview, physical examination, and laboratory tests. The risk of CHD and the diagnosis of metabolic syndrome were assessed according to the special criteria. Obese patients were identified and the incidence of arterial hypertension and diabetes was assessed. The patients were classified as very low / low and moderate / high risk according to the risk of progression of CKD and its associations with arterial hypertension, diabetes mellitus, metabolic syndrome, obesity and cardiovascular risk were identified. Results: Moderate to high chronic kidney disease progression (CKD) risk was three times more likely to develop for older patients compared to those, who were younger than 65 years old, 20,2% and 6,7% respectively (p=0,005). High CKD progression risk was more than twice the incidence among those with diabetes compared to those without, 22,6% and 9,6% respectively (p=0,016). Obesity was multiplicatively associated with CKD progression risk, it was 4 times higher for obesed patients, compared to those who body mass index was lower than 30 kg/m2 (21% and 5,1% respectively) (p=0,010). Respondents who had high risk of cardiovascular disease (CVD) were 17,5 times more likely to develop moderate to high risk of CKD progression, compared to those who had low CVD, risk 21% and 1,2% respectively (p < 0,0001). Significant links between sex, hypertension, metabolic syndrome and CKD progression risk were not found. Conclusions: Significant links between arterial hypertension, metabolic syndrome and CKD progression risk were not found in this study. CKD progression risk was signicically associated with with diabetes and obesity. A strong link between progression risk of CKD and cardiovascular disease was also found
