Rheumatoid factor IgE in juvenile rheumatoid arthritis

Juvenile Rheumatoid Arthritis (JRA) is a chronic inflammatory disease whose pathogenic mechanism is still unclear. The clinical manifestations of this disease result from a series of multifactorial events involving mechanisms dependent on B and T cells. The IgM Rheumatoid Factor (FR-lgM) detected by the latex agglutination technique occurs in 5% to 10% of patients with JRA and it is associated with progression to erosive and disabling disease. Some studies show that other RF isotypes (IgA, IgG and IgE) may be present in this disease, although its clinical and prognostic value remains unknown. The objectives of the present study were: to determine the presence of FR-IgE in the FYR and correlate it with prognostic factors. A prospective multicenter study was carried out between January 1993 and January 1999 with the participation of three pediatric freumatology centers (Faculty of Medicine, University of São Paulo, Ribeirão Preto School of Medicine, University of São Paulo and Federal University of Uberlândia ). 91 children diagnosed with FRA were studied according to the criteria of the American College of Rheumatology: 38 (42%) with the systemic onset, 28 (31%) pauciarticular and 25 (27%) polyarticular. The average age was 10 years and 6 months, with a range from 2 years and 1 month to 22 years and 7 months and 59 (65%) children were female. The control group consisted of 45 healthy children. The detection of FR-lgE was performed through an immunoenzymatic assay (ELISA). Mouse IgG was used to sensitize microplates and, as a conjugate, human anti-IgE labeled with peroxidase. For each reaction, the cut-off value was established according to the average of the optical densities of the negative controls plus three standard deviations. In order to avoid possible intra- and inter-assay variations, a correction factor was established to express the results in terms of the ELISA index. FR-lgE was correlated with: FR-lgM (latex), total serum IgE, ESR, ANA, functional and radiological class III or IV. Of the 91 children with JRA, 15 (16.5%) had positive FR-IgE. Of these, 7 (18.5%) in the systemic form, 5 (18%) in the pauciarticular and 3 (12%) in the polyarticular. There was a statistically significant correlation between FR-lgE and: geometric mean of total serum IgE in the total of patients with JRA and positivity of FR-lgM (latex) in patients with the polyarticular shape (p = 0.0312). There was no statistical correlation between FR-IgE and functional and radiological class III or IV in any form of disease onset. Of the 45 controls, 5 (11%) also showed positive FR-IgE, but there was no correlation with total serum IgE. In conclusion, FR-lgE can be detected in 16.5% of patients with JRA, especially in those with high levels of total serum IgE. The presence of FR-lgE in patients with polyarticular shape is also generally correlated with positivity for FR-lgM (latex). FR-lgE does not seem to be associated with a worse prognosis for JRA.Dissertação (Mestrado)A Artrite Reumatóide Juvenil (ARJ) é uma doença inflamatória crônica cujo mecanismo patogênico ainda não está esclarecido. As manifestações clínicas desta doença resultam de uma série de eventos multifatoriais que envolvem mecanismos dependentes de células B e T. O Fator Reumatóide IgM (FR-lgM) detectado pela técnica de aglutinação em látex ocorre em 5% a 10% dos pacientes com ARJ e associa-se com evolução para doença erosiva e incapacitante. Alguns estudos mostram que outros isotipos do FR (IgA, IgG e IgE) podem estar presentes nesta doença, embora o seu valor clínico e prognóstico permanece desconhecido. Os objetivos do presente trabalho foram: determinar a presença do FR-lgE na ARJ e correlacioná-la com fatores prognósticos. Realizou-se um estudo multicêntrico prospectivo entre janeiro de 1993 a janeiro de 1999 com ^participação de três centros de freumatologia pediátrica (Faculdade de Medicina da Universidade de São Paulo, Faculdade de Medicina de Ribeirão Preto da Universidade de São Paulo e Universidade Federal de Uberlândia). Estudaram-se 91 crianças com o diagnóstico de ARJ de acordo com os critérios do Colégio Americano de Reumatologia: 38 (42%) com a forma de início sistêmica, 28 (31%) pauciarticular e 25 (27%) poliarticular. A idade média foi de 10 anos e 6 meses, com variação de 2 anos e 1 mês a 22 anos e 7 meses e 59 (65%) crianças eram do sexo feminino. O grupo controle constituiu-se de 45 crianças sadias. A detecção do FR-lgE foi realizada através de um ensaio imunoenzimático (ELISA). Utilizou-se IgG de camundongo para a sensibilização das microplacas e, como conjugado, anti-lgE humana marcada com peroxidase. Para cada reação foi estabelecido o valor do cut-offj segundo a média das densidades óticas dos controles negativos mais três desvios-padrões. Para evitar possíveis variações intra- e interensaiosfoi estabelecido um fator de correção para expressar os resultados em termos de índice-ELISA. O FR-lgE foi correlacionado com : FR-lgM (látex), IgE sérica total, VHS, FAN, classe funcional e radiológica III ou IV. Das 91 crianças com ARJ, 15 (16,5%) apresentaram FR-lgE positivo. Destas, 7(18,5%) na forma sistêmica, 5 (18%) na pauciarticular e 3 (12%) na poliarticular. Observou-se correlação estatisticamente significativa entre o FR-lgE e: média geométrica da IgE sérica total no total dos pacientes com ARJ e a positividade do FR-lgM (látex) nos pacientes com a forma poliarticular (p= 0,0312). Não foi observada correlação estatística entre o FR-lgE e classe funcional e radiológica III ou IV em nenhuma das formas de Início da doença. Dos 45 controles, 5 (11%) também apresentaram FR-lgE positivo, mas não houve correlação com a IgE sérica total. Concluindo, o FR-lgE pode ser detectado em 16,5% dos pacientes com ARJ, especialmente naqueles com níveis elevados de IgE sérica total. A presença do FR-lgE em pacientes com forma poliarticular correlaciona-se, em geral, também com a positividade para o FR-lgM (látex). O FR-lgE parece não se associou com pior prognóstico da ARJ
