Transgender health care: decisions about transgenitalization surgery among transsexual women

Transsexuality is an important theme in the field of gender studies, representing an area of constant theoretical and practical disputes between researchers, academics and activists. In Brazil, assistance to transsexual people in the public health service is structured through the Ministry of Health Ordinances that defined and expanded the Transsexualizing Process. Among the possibilities of procedures in the Transsexualizing Process, transgenitalization surgery has been an important part of the transition processes of some transsexual women. From a theoretical point of view, studies linked to Queer Theory are substantial in understanding transsexuality from a depathologizing conception of the transsexual experience and a critical view of social gender structures. Based on these points, the objective of the present study was to identify the interpretative repertoires used in decision processes related to transgenitalization surgery in the Transsexualizing Process among transsexual women who attend the integral health service to the transgender population. To this end, ten semi- structured interviews and audio-video recorded with transsexual women were conducted and these interviews were analyzed from the perspective of discourse analysis proposed by Discursive Psychology. Among the results, the interpretative repertoires identified were divided into three axes: repertoires that question transgenitalization surgery, repertoires that support transgenitalization surgery and repertoires that question and support transgenitalization surgery. The analysis of the repertoires allowed a discussion about the ideas, senses and meanings involved in transgenitalization surgery and made it possible to look at the place that surgery can occupy in the experience of transsexual women.Dissertação (Mestrado)A transexualidade é uma temática importante no campo de estudos do gênero, representando uma área de constantes disputas teóricas e práticas entre pesquisadores, estudiosos e militantes. No Brasil, o atendimento às pessoas transexuais no serviço público de saúde está estruturado por meio das Portarias do Ministério da Saúde que definiram e ampliaram o Processo Transexualizador. Dentre as possibilidades de procedimentos no Processo Transexualizador, a cirurgia de transgenitalização tem sido parte importante na reflexão teórica e na assistência aos processos de transição de mulheres transexuais. Do ponto de vista teórico, os estudos vinculados à Teoria Queer são substanciais na compreensão da transexualidade a partir de uma concepção despatologizante da experiência transexual e de uma visão crítica das estruturas sociais de gênero. A partir desses aportes, o objetivo do presente estudo foi identificar os repertórios interpretativos utilizados nos processos de decisão relativos à cirurgia de transgenitalização entre mulheres transexuais que frequentam um serviço de saúde integral à população transgênero. Para tanto, realizou-se dez entrevistas semi-estruturadas e áudio-vídeo gravadas com mulheres transexuais, de forma que as entrevistas foram analisadas sob a perspectiva de análise do discurso proposta pela Psicologia Discursiva. Dentre os resultados, os repertórios interpretativos identificados foram divididos em três eixos: repertórios que questionam a cirurgia de transgenitalização, repertórios que sustentam a realização da cirurgia e repertórios que questionam e sustentam a intervenção cirúrgica. A análise dos repertórios permitiu uma discussão sobre os sentidos e significados envolvidos na cirurgia de transgenitalização, tanto quanto um olhar para o lugar que a cirurgia pode ocupar na vivência de mulheres transexuais
