Medication reconciliation is the process of comparing a patient's medication orders to all of the medications that the patient has been taking. It is important to obtain an accurate and complete list of medications (Best Possible Medication History), which is based on information provided by structured interview with the patient and other different sources of information, in order to identify and resolve medication discrepancies. Our review included 24 articles, published from 1995 through 2015, that described the effectivness of medication reconciliation regarding to hospital readmissions, emergency department visits and length of stay. Unintentional discrepancies are common and potentially clinically important. Medication reconciliation process with related interventions in therapy improved medication discrepancy and reduced the number of hospital readmissions and emergency department visits. Studies have shown that a pharmacist is ideal for medication reconciliation process and more comprehensive in obtaining medication history than the other health care professionals. The implementation of medication reconciliation in health care system would improve patient safety and decrease costs
