Medication reconciliation is the process of comparing a patient's medication orders to all of the medications that the patient has been taking. It is important to obtain an accurate and complete list of medications (Best Possible Medication History), which is based on information provided by structured interview with the patient and other different sources of information, in order to identify and resolve medication discrepancies. Our review included 24 articles, published from 1995 through 2015, that described the effectivness of medication reconciliation regarding to hospital readmissions, emergency department visits and length of stay.
