Impact of probiotics supplementation on the prevention and alleviation of atopic dermatitis in children

Tematem niniejszej pracy jest systematyczny przegląd badań klinicznych opisujących wpływ suplementacji probiotyków na prewencję i łagodzenie atopowego zapalenia skóry u dzieci. Wstępna część pracy obejmuje ogólną charakterystykę mikroorganizmów oraz mikroflory człowieka. Następnie omówiono bakterie probiotyczne, ich rodzaje oraz mechanizm działania probiotyków w chorobach alergicznych. W kolejnym rozdziale scharakteryzowano przebieg kliniczny atopowego zapalenia skóry (AZS). W ostatnich rozdziałach pracy opisano wyniki i wnioski z analizy wyselekcjonowanych badań klinicznych oraz podsumowanie pracy. Do tego celu wybrano 15 randomizowanych badań klinicznych z bazy danych PubMed z okresu styczeń 2010 – lipiec 2017. Trzy badania kliniczne dotyczyły wpływu suplementacji probiotyków na prewencję AZS, a pozostałe 12 badań klinicznych omawiało wpływ suplementacji probotyków na łagodzenie objawów AZS. W analizowanych badaniach brały udział dzieci do lat 18 z AZS oraz kobiety w ostatnim trymestrze ciąży. Uwzględniono wpływ suplementacji na wartość skali SCORAD (SCORing of Atopic Dermatitis). Na podstawie analizy opisanych w pracy badań klinicznych wywnioskowano, iż suplementacja mieszaniną szczepów probiotycznych może zapobiegać powstawaniu atopowego zapalania skóry oraz łagodzi jego objawy.The subject of this thesis is a systematic review of clinical trials devoted to the impact of probiotics supplementation on the prevention and alleviation of atopic dermatitis in children. The first part of the thesis provides a general outline of microorganisms and microflora in humans. This is followed by a presentation of probiotic bacteria along with their types and the mechanism behind the operation of probiotics in allergic diseases. The next chapter addresses the clinical course of atopic dermatitis (AD). The final chapters of the thesis include a description of the results and conclusions from the analysis of selected clinical trials as well as a thesis summary. For this purpose, 15 randomised clinical trials were selected from the PubMed database, spanning the period from January 2010 to July 2017. Three clinical trials focused on the impact of probiotics supplementation on preventing AD, whereas the other 12 trials concerned the impact of probiotics supplementation on alleviating the symptoms of AD. The analysed trials were participated in by AD children of up 18 years of age and women in their last trimester of pregnancy. The impact of supplementation on the SCORAD (SCORing of Atopic Dermatitis) scale was factored in as well. The conclusion drawn from the analysis of the clinical trials discussed in the thesis was that supplementation utilising a mixture of probiotic strains may prove effective in preventing the occurrence of atopic dermatitis and in alleviating its symptoms
