Recours spontanés aux urgences du Centre Hospitalier Métropole Savoie sans consultation médicale préalable : étude descriptive de la population

Introduction: The number of emergency visits is increasing steadily. The aim of this study was to qualify patient older than 18 years who go to the emergency department on their own and without contact a physician while medical office is open. Method : This was a descriptive, prospective, epidemiological and monocentric study realised in spring 2016 in the emergency department of the Centre hospitalier Métropole Savoie by using a questionnaire. Results : 247 questionnaires have been analysed. The spontaneous consulting population has a 56% majority participation. The average age was 45 years with a sex ratio of 1,57. 71% lived within 20 minutes of the hospital. 62% consulted for the first time. The two main reasons were the pain (37%) and the accidents (28%). Patients consulting for an accident came faster than patient consulting for others reasons (p<0,001). 88% have an attending physician. 30% had previously contacted or consulted a physician and their attending physician in 76% of cases. 45% of patients who had contacted doctor have consulted for a worsening of their symptoms. 25% of patients who hadn’t consulted a doctor would like to have additional investigations and 19% wished specialized doctor opinion. Attending physician main roles were : primary care (58%), continuity of care (58%), problem management according to the context (60%). Conclusion : Our study has a similar population to the Direction de la recherche, des études, de l’évaluation et des statistiques study in 2013. Most of the spontaneous consultations occure without contacting a general practitioner despite most of them have an attending physician but his functions aren’t well-known.Introduction : Le nombre de consultations aux urgences est en augmentation constante. Cette étude visait à caractériser les patients de plus de 18 ans consultant spontanément aux urgences durant les heures ouvrables de cabinet libéral et par leurs propres moyens sans consultation médicale préalable. Méthode : étude épidémiologique, descriptive, prospective, monocentrique aux urgences du Centre Hospitalier Métropole Savoie réalisée au printemps 2016 en journée au moyen d’un questionnaire. Résultats : 247 questionnaires ont été analysés. La population consultant de façon spontanée était majoritaire à 56%.
