Réorganisation d'une unité de production hospitalière de chimiothérapies par une méthode Lean Six Sigma

The hospital pharmacy in turn begins to resort to this type of method derived from industrial engineering in a context of strong economic constraints and high demands in terms of quality, safety and efficiency. In this thesis, the philosophy, methods and tools of the Lean Six Sigma are detailed and applied to a hospital chemotherapy production unit in order to reorganise it. The objective was to redefine, through a participative and collaborative project, the production processes of this unit in order to increase its efficiency while improving the well-being of the employees and their interest in their work. The DMAICS method was applied as well as the Lean tools and methods such as value stream mapping, waste search, visual management, just-in-time, self-quality, agility, standardisation and simplification. The project resulted in a smooth restructuring of the unit to the satisfaction of all. Processing times have declined, as have stocks, outstandings, movements and stress. Communication within the team has improved, and continuous improvement is gradually taking root. New potential activities with higher value-added have been revealed, so there is still some development potential. This thesis strives to report as widely the transposition of the Lean Six Sigma to a hospital chemotherapy production unit in all its variety of tools, methods and concepts with convincing results.Le Lean Six Sigma connaît ces dernières années un très fort engouement dans tous les secteurs de la santé. La pharmacie hospitalière à son tour commence à recourir à ce type de procédé issu du génie industriel dans un contexte de contraintes économiques fortes et d’exigences élevées en termes de qualité, de sécurité et d’efficience. Dans cette thèse, la philosophie, les méthodes et les outils du Lean Six Sigma sont détaillés puis appliqués à une unité de production hospitalière de chimiothérapie afin de la réorganiser. L’objectif était de redéfinir, à travers un projet participatif et collaboratif, les processus de production de cette unité afin d’améliorer leur efficience tout en améliorant le bien-être des collaborateurs et l’intérêt pour leur travail. La méthode DMAICS a été appliquée ainsi que les outils et méthodes du Lean comme la cartographie de la chaîne de valeur, la recherche des gaspillages, le management visuel, le juste à temps, l’auto-qualité, l’agilité, la standardisation et la simplification. Le projet a abouti à une restructuration douce de l’unité à la satisfaction de tous. Les temps de réalisation des processus ont diminué, tout comme les stocks, les encours, les déplacements et le stress. La communication au sein de l’équipe s’est améliorée et l’amélioration continue s’enracine au quotidien. Un potentiel a également été identifié pour développer l’unité dans le sens d’activités à plus forte valeur ajoutée. Cette thèse s’est efforcée de rendre compte de façon aussi large que possible de la transposition du Lean Six Sigma à une unité de production de chimiothérapies dans toute sa panoplie d’outils, de méthodes et de concepts avec des résultats probants
