Prévalence de l'hypoglycémie en vraie vie chez des patients diabétiques de type 2 ayant suivi un programme d'éducation thérapeutique (ProxYdiab 38)

The actual frequency of hypoglycemia in patients with type 2 diabetes, is not well known. The objective of this work is to evaluate the prevalence of these events over a period of 6 months, in patients with type 2 diabetes, who followed a program of therapeutic education (ProxYdiab 38) from 2011 to 2014 in the urban area of Grenoble. The following elements were also analyzed: quality of sugar intake, impact of a therapeutic education class on the prevention and management of hypoglycemia, the evocation of these events to a doctor, and the psychological and behavioral consequences on patients by the HFS - II scale. The sample tested consists of 65 patients; 36,9% of them have experienced at least one hypoglycemic episode, among them 67% were insulin-treated. Two patients did an episode without any hypoglycemic treatment. In extrapolating these figures, it appears that there are 2.84 hypoglycemic episodes per patient/year. These numbers are not trivial, because based on the analysis of secondary criteria, only 54.1% of the affected patients have had an appropriate sugar intake for a quick correction of their low blood sugar level, and only 37.5% of them have talked about it with their doctor. Moreover, suffering from hypoglycemia also has a psychological and behavioral statistically significant impact even though these episodes are said to be minor. Therefore, hypoglycemia concern a sizeable share of type 2 diabetic patients, and have a real impact on their behavior, as well as an important economic weight for society. Their prevention and medical attention appear to be perfectible; it is therefore essential that healthcare providers foremost source of information of patients are made aware of this issue.La fréquence des hypoglycémies, en vraie vie, chez les patients diabétiques de type 2, est mal connue. L’objectif de ce travail est d’évaluer la prévalence de ces évènements sur une période de 6 mois, chez les patients diabétiques de type 2, ayant suivi un programme d’éducation thérapeutique sur l’agglomération grenobloise entre 2011 et 2014 (association ProxYdiab 38). Etaient aussi évalués : la présence et la qualité du resucrage, l’impact d’un atelier d’éducation thérapeutique concernant la prévention et la gestion des hypoglycémies, l’évocation de ces évènements à un médecin, ainsi que les conséquences psychologiques et comportementales sur les patients, par le score HFS-II. L’échantillon analysé est constitué de 65 patients ; 36,9% d’entre eux ont présenté au moins un épisode hypoglycémique ; parmi eux 67% étaient insulino-traités. Deux patients ont fait un épisode sans traitement connu comme hypoglycémiant. En extrapolant ces chiffres, il apparait qu’il existerait 2,84 épisodes hypoglycémiques par patient/année. Ces chiffres ne sont pas anodins car d’après l’analyse des critères secondaires finalement seuls 54,1% des patients concernés se sont resucrés de façon adaptée pour une correction rapide de leur glycémie ; et uniquement 37,5% d’entre eux ont abordé ou comptaient aborder le sujet avec leur médecin. Par ailleurs le fait de présenter des hypoglycémies a, chez les patients, un impact psychologique et comportemental statistiquement significatif, et ce même si ces hypoglycémies sont dites mineures. Les hypoglycémies concernent donc une part non négligeable des patients diabétiques de type 2, et ont un impact réel sur leur comportement et leur ressenti, ainsi qu'un poids médico-économique important pour la société. Leur prévention et leur prise en charge paraissent perfectibles ; il est donc indispensable que les soignants, sources principales de l'information des patients, soient sensibilisés à cette problématique
