Ductal adenocarcinoma of the pancreatic head and extrahepatic cholangiocarcinoma are the two most common causes of malignant biliary stenosis explored during Endoscopic Retrograde Cholangio-Pancreatography. As much work has been done previously on biliary brushing but few on the cytological examination of bile fluid spreads, our aim was to redefine the morphological cytological criteria for malignancy on bile. We found that the presence of a double cell population, isolated malignant cells, three-dimensional clusters, anisocaryosis, irregularities in nuclear contours, enhancement of nuclear membranes, chromatin distribution anomalies and an increase in the nucleo-cytoplasmic ratio are strong arguments in favour of malignancy, very sensitive...
