Hypervitaminémie B12 : facteur pronostic de morbi-mortalité de la personne âgée ?

The dosage of vitamin B12 is not standardized, however, studies have linked high vitamin B12 levels to the presence of serious illnesses. The main objective was to study in a geriatric population the association between high vitamin B12 levels and the diagnosis of cancer or liver diseases considering the characteristics of the geriatric assessment. The secondary objective was to study the relationship between high vitamin B12 levels and the care pathway the following year. This observational retrospective study included all the patients hospitalized in the acute care for elders unit of the GHM between 01/01/2015 and 12/31/2016. Socio-economic, geriatric assessment and care pathway datas were collected from the medical report and after 1 year from their computerized medical file or by a telephone questionnaire. Patients were considered lost to follow-up when 1-year datas were not available. The main judgement criterion was the presence at 1 year of solid neoplasia, malignant blood or liver diseases. 397 patients were hospitalized, 25 were excluded because not tested. 233 files were analyzed because 139 were considered lost to follow-up. Univariate study showed that patients with high vitamin B12 levels had higher CIRS-G (p=0.01) and were frequently readmitted within the following year (p=0.03). Only the presence of cancer or liver disease remained significantly associated with high vitamin B12 levels considering the multivariate analysis (OR 6.23 95%CI [2.58;15.02]). In this population, high vitamin B12 levels were associated with the presence of serious diseases regardless of the geriatric assessment and appears to be a risk factor for readmission at 1 year, but not for mortality.Le dosage de la vitamine B12 ne fait pas consensus, pourtant des études ont associé l’hypervitaminémie B12 à la présence de maladies graves. L’objectif principal étudiait dans une cohorte âgée l’association entre hypervitaminémie B12 et les diagnostics de cancer ou d’hépatopathie au regard des paramètres de l’évaluation gériatrique. L’objectif secondaire était d’étudier la relation entre hypervitaminémie B12 et le parcours de soins. Cette étude observationnelle rétrospective, incluait tous les patients admis en court séjour gériatrique au GHM entre le 01/01/2015 et le 31/12/2016. Les caractéristiques socio-économiques, de l’évaluation gériatrique et du parcours de soin ont été recueillies à partir du RUM puis à 1 an à partir de leur dossier médical informatisé ou d'un questionnaire téléphonique. Ils ont été considérés comme perdus de vue si les données à 1 an n’étaient pas disponibles. Le critère de jugement principal était la présence à 1 an d’une néoplasie solide, d’une hémopathie maligne ou d’une hépatopathie. 397 patients ont été hospitalisés, 25 ont été exclus car non dosé. On comptait 233 dossiers analysables, 139 ont été considérés perdus de vues. En univariée les patients avec une hypervitaminémie B12 avaient un CIRS-G plus élevé (p=0,01) et étaient plus réadmis avant 1 an (p=0,03). En multivariée seule la présence d’un cancer ou d’une hépatopathie restait significativement associée à une hypervitaminémie B12 (OR 6.23 IC95% [2.6;15.0]). L’hypervitaminémie B12 était liée à la présence de pathologies graves indépendamment des paramètres de l’évaluation gériatrique et semblerait être un facteur de risque de réadmission mais pas de surmortalité
