Description de l’organisation de la cinquième année hospitalo-universitaire en France et de sa perception par les étudiants dans un contexte de réforme des études de pharmacie

Introduction and objective: In France, the fifth year of PharmD program is called “university hospital year” because an internship in hospital must be completed by students during this period. There is a national pedagogical guide for clinical pharmacy hospital functions to help students in their duties performance. Recently, the Deans’ Conference of the Faculties of Pharmacy of France has published a guide for pharmacy students about their hospital functions and training objectives. Because of the heterogeneity of this training year configuration in the 24 faculties and the current context of reform, we wanted to evaluate student perception and describe organization of this training year. Methods: During March and April 2019, all fifth and sixth year pharmacy students in France (n=6196) were interviewed using an on-line questionnaire that included seven sections: (1) interest and education, (2) supervision, (3) role and (4) activities during the internship, (5) interprofessionnalism, (6) knowledge of the existing pedagogical guide and (7) global appreciation. To describe organization, in each faculty, the referent teachers of this training year (n=24) were also interviewed by an on-line questionnaire arranged in four sections: (1) preparation of university hospital year, (2) hosting of student, (3) supervision and (4) assessment of the internship. A multiple correspondence analysis and hierarchical clustering were also performed to identify different student profiles. Results: Respectively 24 (100%) and 836 (13.5%) responses were obtained from teachers and students. We identified disparities between faculties for students’ preparation and assessment of the internship. Pharmaceutical dispensing products and clinical pharmacy are the two field with the most responses “most agree” and “totally agree”. Indeed students feel for those fields more attracted, more trained, more supervised, integrated and useful. Among sixth-year students, 37 (13.5%) and 30 (10.9%) indicated performing case presentation and critical reading article more than 5 times during the internship. 818 (97.8 %) students didn’t have the knowledge of the pedagogical guide. Moreover, 7 (29.2%) faculties have position description for all the internships they propose. Conclusion: Students perception about this training year and its internship can be improved and this survey could help renovating the national pedagogical guide and it could help developing the training objectives’ guide.Le stage hospitalier de 5ème année hospitalo-universitaire (5AHU) permet à l’étudiant en pharmacie d’exercer des fonctions centrées sur le patient, en relation avec l'équipe médicale. La configuration hétérogène de la 5AHU au niveau national, associée aux actuelles réformes des études de pharmacie et au faible nombre d’études sur ce sujet, nous a incités à évaluer sa perception par les étudiants et son organisation au sein des 24 facultés de Pharmacie de France. Dans chaque faculté, les enseignants responsables de la 5AHU et les étudiants de 5ème et 6ème année ont été interrogés en Mars-Avril 2019 via un questionnaire électronique. Respectivement 24 (100%) et 836 (13.5%) réponses ont été obtenues pour les questionnaires enseignant et étudiant. La majorité des étudiants étaient d’accord ou tout à fait d’accord pour dire que la 5AHU conforte le choix de filière (69.3%) et constitue une aide pour la pratique professionnelle future (56.6%). Notre étude montre cependant que des efforts supplémentaires peuvent être fournis pour améliorer l’encadrement, l’intégration ou les rôles de l’étudiant durant les stages effectués dans les différents domaines de pharmacie hospitalière. De même les cours et modalités de validation de la préparation à la prise de fonctions hospitalière ainsi que les modalités de validation des stages présentent des disparités selon les facultés. En perspective, la rénovation du guide pédagogique des fonctions hospitalières de pharmacie clinique existant ou une évolution du guide de compétence proposé par la Conférence des Doyens des Facultés de Pharmacie de France pourraient être envisagées
