Predictable adverse clinical findings for the development of liver fibrosis in patients with chronic liver disease

Ne – alkohola steatohepatīts (NASH) (no angļu val. non-alchoclic steatohepatitis; NASH) raksturojas ar iekaisumu aknās un paaugstinātu fibrotisko audu veidošanos un aknu nepietiekamības progresiju. Pašlaik nav noteikti konkrēti aknu fibrozi veicinošie faktori. Lai apstiprinātu diagnozi, vienīgā precīzā metode ir aknu biopsija, taču metode ir invazīva un nevar tikt izmantota ikdienas praksē. Pētījumos līdz šim cukura diabēts, aptaukošanās, vecāks vecums un aknu enzīmu marķieri tiek izmantoti aknu fibrozes progresijas noteikšanai. Darba mērķis ir atrast un izvērtēt Latvijas iedzīvotāju iespējamos riska faktorus slimības attīstībai un aknu fibrozes progresijai. Hipotēze: Aknu fibrozes attīstība ir atkarīga no slimības ilguma, ko veicina neregulāri un uzturvielu ziņā nepietiekami ēšanas paradumi, nepietiekama fiziskā aktivitāte brīvajā laikā un slikta miega higiēna. Hroniski noritošu aknu slimību gaitu ietekmē antioksidantu un oksidantu līdzsvara zudums uzturā. Metodes: Prospektīvs pētījums, kura ietvaros tika veiktas intervijas ar pacientiem (n=36) no Rīgas Austrumu klīniskās universitātes slimnīcas “Gaiļezers” ambulatorās daļas ar diagnosticētu NATAS. Intervijas laikā tika noskaidroti uztura un dzīvesveida paradumi. Pēc tam pacienti tika aicināti pierakstīt trīs dienu uztura dienasgrāmatas, kuru dati tika analizēti ar pārtikas sastāva datu bāzi. Iegūtie dati tika analizēti un apskatīti saistībā ar aknu fibrozes progresijas raksturojošiem marķieriem no pieejamiem izmeklējumu datiem no pacientu slimības vēsturēm. Rezultāti: Saldināto dzērienu regulāru lietošanu novēro vīriešiem (p = 0,007). Jaunākie pētījumā iekļautie pacientie ikdienā saldinātos dzērienus (p = 0,001) un svaigu sarkano gaļu (p = 0,012) lieto regulāri. Alanīnaminotransferāze (ALT) regulāru un neregulāru sarkanās gaļas lietotāju vidū bija būtiski atšķirīga (p = 0,032). ALT mediāna gaļas lietotājiem 65,00 U/L (38,50 U/L– 83,50 U/L) pretēji nelietotāju izlasē 30,00 U/L (20,00 U/L – 60,00 U/L). Vara dauduzms uzturā bija augstāks zemākās aspartātaminotransferāzes (AST) un ALT attiecības grupā (p = 0,025). Vīriešiem ir palielināts gurnu apkārtmērs augsta riska fibrozes attīstības grupās - gan AST/ALT, gan BARD riska grupās (p = 0,030; p = 0,001). Sievietēm 63,2% gadījumu ir pozitīva aknu steatozi veicinošu medikamentu lietošanas anamnēze un no visiem cukura diabēta pacientiem 68,8% ir sievietes. Vecuma mediānas ir būtiski atšķirīgas medikamentu pozitīvā grupā - 54 gadi, pretēji nelietotāju grupā 40 gadi (p = 0,003). Vidukļa apkārtmērs ir statistiski nozīmīgi atšķirīgs vecuma grupās (p = 0,005). Pacientēm bez medikamentu lietošanas anamnēzes vidukļa apkārtmēra mediāna bija 112,00 cm (103,00 – 118,00) pretēji sievietēm ar pozitīvu medikamentu anamnēzi vidukļa apkārtmēra mediāna 99,50 cm (93,25 – 103,25). Secinājumi: Aknu fibrozes progresiju veicina uztuIntroduction: Non-alcoholic fatty liver diesease (NAFLD) affects 25-26% of the population in Europe. Non-alcoholic steatohepatitis (NASH) is characterized by inflammation of the liver and increased fibrotic tissue formation and progression of liver failure. Specific factors contributing to the progression of the disease to fibrosis and hepatic insufficiency have not yet been identified. Liver biopsy is the only accurate method to confirm the diagnosis, but the method is invasive and cannot be used in daily practice. In studies diabetes, obesity, older age, liver enzyme markers have been used to measure the progression of liver fibrosis. The aim of this work is to find and evaluate the potential risk factors for the development of the disease and the progression of liver fibrosis in the population of Latvia. Hypothesis: The development of liver fibrosis depends on the duration of the disease, which is caused by irregular and not healthy eating habits, poor physical activity and poor sleep. Chronic liver disease is affected by the loss of antioxidant and oxidant balance in the diet. Methods: Prospective study, which included interviews with patients (n = 36) from the out-patient department of Riga Eastern Clinical University Hospital “Gaiļezers” diagnosed with NAFLD. During the interview, diet and lifestyle habits were explored. Patients were asked to write down a three-day diet diary, the data were analyzed with a food nutrition content database. The data obtained were analyzed and the association with markers of progression of liver fibrosis from available medical history of patients were reviewed. Results: Regular use of sweetened drinks was observed in men (p = 0.007). The youngest patients enrolled in the study consume daily sweetened drinks (p = 0.001) and fresh red meat (p = 0.012). Alanine aminotransferase (ALT) is significantly different between regular and non-regular red meat users (p = 0.032). ALT median in meat users 65.00 U/L (38.50 U/L - 83.50 U/L) versus non-user sample 30.00 U/L (20.00 U/L - 60.00 U/L). Dietary copper is higher in the lower ratio of aspartate aminotransferase (AST) to ALT (p = 0.025). In the high-risk fibrosis development group, men have increased hip circumference in both AST/ALT and BARD risk groups (p = 0.030; p = 0.001). In women, 63.2% of cases have a positive history of liver steatosis inducing medication use and 68.8% of all diabetes patients are women. The median age in the drug positive group was 54 years, compared with 40 years in the non-users group (p = 0,003). Waist circumference is statistically significantly different between age groups in these women (p = 0.005). The median waist circumference in women patients without medication use were 112.00 cm (103.00 - 118.00) as compared to waist circumference for women with positive medication 99.50 cm (93.25 - 103.25).
