Tacrolimus (Tac), a macrolide lactone produced by the fungus Streptomyces tsukubaensis, is nowadays the primary immunosuppressive drug used in organ transplantation. As cyclosporine, Tac is a calcineurin inhibitor preventing cellular rejection through the selective inhibition of interleukin-2 production by T-cells. Immunosuppressive agents are critical dose drugs; this means they have a narrow therapeutic index, exhibiting the desired therapeutic effect with acceptable tolerability only within a narrow range of blood concentrations.
