Lateral nazal duvarın endoskopik topografik anatomisi ve riskli bölgeler (Kadavra diseksiyonu)

By the cadaveric dissection is made to go towards endoscopic sinus surgery to examine the lateral nasal wall topographic anatomy. During the dissection by five cadaver, the lateral nasal wall anatomy, probable anatomic variations, suitable surgical landmarks, this structures distant of constant points and the high-risk areas are examined. By the dissections which we made by one of the case concha bullosa, an another case supreme nasal concha and by the another case agger nasi cells determined. Anterior attachment of the middle concha, superior border of choana, anterior nasal spine and upper surface of the inferior concha are found out as surgical landmarks. Lamina papyracea, etmoid roof near the anterior ethmoid artery, lateral lamella of the cribriform plate, ethmoid roof near the posterior ethmoid artery and upper two-third of the lateral wall of the sphenoid sinus are founded out as risk area. The result of the measurment of lateral nasal wall can help surgeon by endoscopic sinus surgery to found out the localization, but they are not safe enough to be used alone.Bu çalışmada endoskopik sinüs cerrahisine yönelik lateral nazal duvarın topoğrafik anatomisini incelemek için kadavra diseksiyonu yapılmıştır. Beş kadavra üzerinde yapılan diseksiyon sırasında; lateral nazal duvarın anatomisi, olası anatomik varyasyonlar, nirengi noktası olabilecek yapılar, bu yapıların sabit noktalardan uzaklıkları ve riskli bölgeler incelenmiştir. Yaptığımız diseksiyonlarda anatomik varyasyon olarak bir vakada konka bülloza, bir vakada supreme konka ve bir vakada agger nazi tespit edilmiştir. Nirengi noktalan olarak orta konka ön çıkıntısı, koana üst sının, anterior nazal spin ve alt konkanın üst yüzü belirlenmiştir. Lamına papyracea, anterior etmoid artere komşu etmoid sinüs çatısı, cribriform plate lateral lamellası, posterior etmoid artere komşu etmoid çatı ve sfenoid sinüs lateral duvannın 2/3 üst bölümü riskli bölge olarak tanımlanmıştır. Lateral nazal duvarda yapılan ölçümlerde elde edilen sonuçlann endoskopik sinüs cerrahisinde cerraha lokalizasyonunu belirlemede yardımcı olacağı ancak tek başına güvenilir olmadıklan bulunmuştur
