Psychological distress and personality traits predictors associated with information needs, decision making preferences and quality of life of breast cancer patients

In recent years there has been an increasing emphasis on patients’ autonomy and there has beenmuch debate over their participation in medical decision making. Patients’ roles in the decisionmaking process range from playing an active role, i.e. patients decide themselves about their treatment, through a sharing role, to a passive role in which they delegate responsibility of thedecision to their physicians. Shared decision making is increasingly advocated as an ideal model of physician–patient interaction. Although it is not yet clearly understood what role patients really want to play in decision making, seeking information and being involved in decision making are thought to contribute to coping processes, which in turn promote the adjustment to the disease, at a time when patients experience significant levels of psychological distress. Breast cancer is considered an appropriate type of cancer for studying decision-making processes, because its treatment involves difficult choices between medically justified treatment alternatives. For example, research indicates that both mastectomy and breast conserving surgery with radiation offer equivalent survival rates in early stages of breast cancer. Even though in Greece, in recent years, the choice of treatment is discussed with some patients depending on their educational level and health status, paternalism is not yet challenged enough. Physicians often decide on their own the most appropriate treatment, without previously having informed the patient in detail about their illness, available treatment options or prognosis. On the other hand, a number of studies have investigated clinical, sociodemographic and psychosocial factors which could predict psychological distress and quality of life of patients with breast cancer. Sociodemographic and disease-related variables such age, time since diagnosis, cancer stage, physical functioning, fatigue, perceived social support and copingstrategies, as well as psychological factors, such psychiatric history of affective disorders, andpersonality traits such as optimism and trait anxiety, were found to be predictive of later psychological distress and of quality of life for women with breast cancer. There is, however, little research on the predictive power of psychodynamic parameters such as ego mechanisms of defense in breast cancer patients’ HRQoL. In addition no studies have investigated their relationship with decisional preferences in breast cancer. The coping process theory assumes that involvement in treatment decision making may be a coping response to perceived threats, challenges and harms associated with cancer and its treatment. On the other hand, cancer diagnosis is a severe psychological stressor, and the patients’ predominant defenses may determine their psychological response. Hence, defense mechanisms may influence breast cancer patient’s decisional preferences and HRQoL. Therefore, we aimed to: (i) explore the information needs and decision making preferences of Greek women with breast cancer, (ii) identify psychological factors, namely, state and trait anxiety, depressive symptoms and defense mechanisms associated with decisional preferences in women with nonmetastatic breast cancer, assuming that avoidance - prone defenses, such as denial or repression, are associated with passive decisional preferences, (iii) assess the course of patients’ psychological distress and HRQoL in a prospective study over a 1-year period and (iv) identify independent predictors of change in psychological distress and HRQoL. In order to study breast cancer patients’ information needs and decision making preferences we performed a cross sectional study design on a sample of all women with a confirmed diagnosis of breast cancer who were being treated or followed up in the oncological outpatient clinic of the Ioannina Teaching General Hospital from February 2008 to September 2009. When it came to exploring patients’ quality of life and its correlations with decision making preferences, psychological distress and ego defense mechanisms, we focused on a sub- sample of non – metastatic patients and we adopted a longitudinal study design. Cross sectional study. Aims: We aimed to assess Greek breast cancer patients’ preferences for participation in treatment decision making and their information needs.Methods: In 329 Greek breast cancer patients the “Control Preferences Scale”, i.e. a card-sortmeasurement designed to elicit preferences for participation in decision making, was administered. Information needs were assessed with the Cassileth’s Information Styles Questionnaire. Mean age was 59.5 (SD, 10.9) years. Mean disease duration was 43.37 (SD, 53.7) months. The majority had stage II disease at the time of diagnosis (53.5%) and had been treated with mastectomy (69.9%).Results: The majority of patients (71.1%) preferred to play a passive role in treatment decisionmaking, with most of them wanting to delegate responsibility of the decision completely to their doctor (45.3%). A collaborative role was preferred by 24%, whereas only 4.6% chose an active role. Most women expressed a general desire for as much information as possible about their illness (62.6%), but a substantial proportion (37.4%) did not want detailed information; instead, they wished to avoid awareness of bad news. Women who desired less informational details and preferred a passive role had requested less frequently a mammography (p<0.001) and/or Pap test (p<0.0005) prediagnostically. Prospective Study. Aims: We aimed to assess psychological correlates of treatment decisional preferences and predictors of HRQoL in women with non-metastatic breast cancer. Methods: One hundred and twenty-four women with non-metastatic breast cancer completed baseline questionnaires for anxiety (Spielberger’s State–Trait Anxiety Inventory) anddepressive symptoms (Center for Epidemiologic Studies-Depression (CES-D), HRQoL (WHOQOL-BREF), and defense mechanisms (Life Style Index). Of them, 82 (66.1%) completed the 1-year follow-up. Mean age was 54.6 years (SD = 9.76), and mean disease duration was 19.4 months (SD = 25.55); 19.5% had stage I, 63.4% stage II and 17.1% stage III disease. The predictive power and moderator effects of psychological variables were tested using multiple and hierarchical regression models. Results: Depressive symptoms and physical HRQoL improved significantly, state anxiety and mental and environment HRQoL remained stable, and social relations HRQoL deteriorated over the 1-year period. Older age (p = 0.021) and higher scores in repression defense (p = 0.044) were independently associated with passive decisional preferences. Earlier stage ofcancer (p = 0.043), lower state anxiety (p = 0.039), lower repression scores (p = 0.021) and improvement in depressive symptoms (p < 0.001) predicted physical HRQoL improvement. Moderation analysis showed that active decisional preferences predicted physical HRQoL improvement, but only in those women with lower repression levels. Conclusions. Our cross sectional study’s findings showed that the proportion of patients who wanted to play a passive role in decision making is the highest reported compared to similar studies from other countries, indicating the impact of the dominating paternalistic model of the doctorpatient relationship in the Greek medical encounter. Importantly, women who desired less informational details and preferred a passive role had requested less frequently a mammography and/or Pap test prediagnostically.The prospective study showed a decrement in the levels of depressive symptoms and improvement in Physical HRQoL in women with early non-metastatic breast cancer over a period of one year. Social relations HRQoL, however, deteriorated during the same time period. A preference for a passive role in decision making was strongly associated with the patients’ resources to deal with stressors, especially with repression defense. Repression, along with initial levels of HRQoL and improvement in depressive symptoms, was also a significant independent predictor of most aspects of HRQoL, while moderator analysis showed that an active decisional preference predicted improvement in Physical HRQoL, but only in those women with lower levels of repression defense. To our knowledge, this is the first study indicating that repression defense predicts HRQoL and is associated with decision making preferences in breast cancer patients. Strengths and limitationsCross sectional Study: The strengths of the study include the use of widely used instruments which have been adapted to Greek following thorough procedures. We selected a rather large sample of patients with high response rate (90.4%) and we found no statistically significant differences between responders and non responders. Moreover, our hospital provides care to the majority of the breast cancer patients of the catchment area, suggesting that we recruited a representative sample of women with breast cancer. With regards to our study’s limitations: 1) our sample comprised patients with a wide range of disease duration with a variety of stages of the disease; 2) The results about whether women with stage I or II breast cancer perceived that they had a choice between mastectomy and breast conserving treatment with radiation therapy were based on the patients’ subjective experience. Interactions between health-care professionals and patients were not directly observed; 3) Our study was focused on the doctor–patient relationship and did not include patients’ relatives, who often play a significant role in clinical consultation, and 4) the crosssectional design of our study (for example, patients’ decisional preferences may change over time, as their experience/ knowledge about illness grows and their relationship with their doctors strengthens). Prospective Study: Strengths: The in-depth psychological evaluation, the use of well recognized standardized instruments and the prospective design are major strengths of this work.Limitations: 1) We excluded women with metastases, and this positive selection bias prevents the generalization of our findings to other breast cancer patients; 2) We recruited patients at any point of their disease course and, consequently, the ‘baseline’ level was individual; 3) We assessed patient’s preferred decisional role and not the role that they actually played in treatment decisional preferences, and 4) The relatively high number of drop-outs at follow-up may have impacted our findings, although it is unlikely given that there were no statistically significant differences in the preferred decisional roles and probable depression at baseline between the participants and those who did not participate at follow-up. Clinical Implications: Physicians should give adequate information to women diagnosed with breast cancer about the stage of disease, available treatment options, their side effects and potentially their prognosis, particularly to women who express the desire to get detailed information, for example by asking a lot of questions. They should also encourage women to play their preferred role in decision making, perhaps by having explored in advance their decision making preferences by using a brief version of CPS. Furthermore, clinicians should identify and manage depression early, perhaps by providing a self- administered questionnaire [such as the CES-D, Becks depression inventory or HAM-D], and referring or discussing with the psychiatric liaison service when a patient scores above the cut off point. Clinicians should also pay attention to patients’ anxiety symptoms as we found that state anxiety may be an important predictor of outcome. Finally, our results indicate that clinicians should also consider the patients’ psychological resources and coping capacities to deal with the disease’s consequences. Frequent missing of scheduled treatment appointments due to ‘forgetting’, ‘forgetfulness’ regarding medical advice, or frequent lapses of the tongue and ‘inattention’ are possible indicators of repression, and clinicians should suspect the activation of repression defense when these phenomena appear and should refer patients for psychological evaluation and management. Future research should explore further the impact of the complex interplay betweenpersonality, psychological distress and disease parameters on patients’ HRQOL. This might provide key targets to address the potential psychological parameters that need to be treated in order to improve breast cancer patients’ HRQOL.Τα τελευταία χρόνια έχει δοθεί ιδιαίτερη έμφαση στην αυτονομία των ασθενών και έχει γίνει πολλή συζήτηση για τη συμμετοχή τους στη λήψη θεραπευτικών αποφάσεων. Ο ρόλος των ασθενών στη λήψη αποφάσεων μπορεί να κυμαίνεται από ενεργός (όπου ο/η ασθενής λαμβάνει μόνος/η του/της την απόφαση) στο συνεργατικό (όπου η απόφαση λαμβάνεται με ισότιμη συμμετοχή γιατρού- ασθενούς) και τον παθητικό (όπου η απόφαση λαμβάνεται εξολοκλήρου από τον/την ιατρό). Η συνεργατική λήψη αποφάσεων (shared decision making) θεωρείται από πολλούς ως ιδεώδες πρότυπο σχέσης γιατρού- ασθενούς. Η απόκτηση πληροφοριών και η συμμετοχή των ασθενών στη λήψη αποφάσεων θεωρούνται στρατηγικές αντιμετώπισης της νόσου που προάγουν την προσαρμογή σε αυτήν. Είναι σημαντικό επομένως να κατανοήσουμε τις επιθυμίες των ασθενών σχετικά με την πληροφόρηση και τη συμμετοχή στη λήψη αποφάσεων, ιδιαίτερα επειδή πολλοί από αυτούς βιώνουν σημαντική ψυχική καταπόνηση. Ο καρκίνος του μαστού θεωρείται κατάλληλος τύπος καρκίνου για να μελετηθεί η διαδικασία λήψης θεραπευτικών αποφάσεων. Αυτό οφείλεται στο ότι υπάρχουν περισσότερες από μία θεραπευτικές επιλογές που θεωρούνται ισότιμες. Παραδείγματος χάριν, η μαστεκτομή και η ογκεκτομή με ακτινοθεραπεία υπολογίζεται ότι είναι ισοδύναμες όσον να αφορά στην επιβίωση για τα πρώιμα στάδια καρκίνου του μαστού. Στην Ελλάδα, αν και τα τελευταία χρόνια η επιλογή της θεραπείας συζητείται με κάποιους/ες ασθενείς ανάλογα με την εκπαίδευση και την κατάσταση της υγείας τους, o πατερναλισμός είναι ακόμα το κυρίαρχο πρότυπο της σχέσης γιατρού- ασθενούς. Οι γιατροί συχνά αποφασίζουν μόνοι τους την πιο κατάλληλη θεραπεία χωρίς προηγουμένως να έχουν ενημερώσει λεπτομερώς τον/ την ασθενή για τη νόσο του/της, τις θεραπευτικές επιλογές, ή την πρόγνωση. Από την άλλη πλευρά, σημαντικός αριθμός μελετών έχουν εξερευνήσει τους κλινικούς, κοινωνικο-δημογραφικούς και ψυχολογικούς παράγοντες που συσχετίζονται με την ψυχική καταπόνηση και την ποιότητα ζωής των ασθενών με καρκίνο του μαστού. Κλινικοί και κοινωνικο-δημογραφικοί παράγοντες όπως η ηλικία, το χρονικό διάστημα που έχει παρέλθει από τη διάγνωση, το στάδιο του καρκίνου, η λειτουργικότητα, η κόπωση, η κοινωνικήυποστήριξη και οι στρατηγικές αντιμετώπισης (coping mechanisms) καθώς και ψυχολογικοί παράγοντες όπως το ιστορικό συναισθηματικής διαταραχής και χαρακτηριστικά της προσωπικότητας όπως η αισιοδοξία (οπτιμισμός) και το άγχος ως χαρακτηριστικό της προσωπικότητας (trait anxiety) βρέθηκε ότι μπορούν να προβλέψουν τα επίπεδα ψυχικής καταπόνησης και την ποιότητα ζωής ασθενών με καρκίνο του μαστού. Ωστόσο, η έρευνα για τη σχέση μεταξύ ψυχοδυναμικών παραγόντων όπως οι μηχανισμοί άμυνας του Εγώ και της ποιότητας ζωής είναι πολύ περιορισμένη. Επίσης, δεν γνωρίζουμε κάποια μελέτη που να έχει επικεντρωθεί στη σχέση του επιθυμητού βαθμού συμμετοχής στη λήψη αποφάσεων και των μηχανισμών άμυνας του Εγώ. Οι θεωρίες για την προσαρμογή στη νόσο υποθέτουν ότι η συμμετοχή στη λήψη απόφασης μπορεί να αποτελεί μηχανισμό προσαρμογής στις απειλές, τις προκλήσεις αλλά και τις σωματικές επιπτώσεις της νόσουαλλά και της θεραπείας της. Από την άλλη πλευρά, το να διαγνωσθεί κάποιος με καρκίνο αποτελεί πολύ ισχυρό στρεσσογόνο παράγοντα και οι μηχανισμοί άμυνας που χρησιμοποιεί μπορεί να καθορίσουν τον τρόπο με τον οποίο χειρίζεται το στρες του. Συνεπώς οι μηχανισμοί άμυνας μπορεί να επηρεάζουν τον επιθυμητό βαθμό συμμετοχής των ασθενών στη λήψη θεραπευτικών αποφάσεων αλλά και την ποιότητα ζωής τους. Έτσι, οι στόχοι της παρούσας μελέτης ήταν: - (1) Να διερευνήσουμε τις ανάγκες πληροφόρησης και τον επιθυμητό βαθμό συμμετοχής στη λήψη θεραπευτικών αποφάσεων των Ελληνίδων που νοσούν από τον καρκίνο του μαστού. - (2) Να διευρευνήσουμε το αν ψυχολογικοί παράγοντες όπως το άγχος, η κατάθλιψη και οι μηχανισμοί άμυνας σχετίζονται με τον επιθυμητό βαθμό συμμετοχής στη λήψη θεραπευτικών αποφάσεων στις γυναίκες με μη-μεταστατικό καρκίνο του μαστού, υποθέτοντας πως οι άμυνες που στρέφουν την ασθενή στην αποφυγή, όπως η άρνηση ή η απώθηση, σχετίζονται με παθητικό επιθυμητό βαθμό συμμετοχής, - (3) Να αξιολογήσουμε πως εξελίσσεται το άγχος, η κατάθλιψη και η Ποιότητας Ζωής που Σχετίζεται με την Υγεία μέσα σε ένα χρόνο σε αυτές τις ασθενείς, και -(4) Να αναγνωρίσουμε τους παράγοντες [δημογραφικούς, κλινικούς, αμυντικούς μηχανισμούς κλπ] που επηρεάζουν την εξέλιξη του άγχους, της κατάθλιψης και της Ποιότητας Ζωής. Προκειμένου να μελετήσουμε τις ανάγκες πληροφόρησης και τον επιθυμητό βαθμό συμμετοχής στη λήψη αποφάσεων υιοθετήσαμε συγχρονικό σχεδιασμό μελέτης. Το δείγμα μας αποτέλεσε το σύνολο των γυναικών με διάγνωση καρκίνου του μαστού που προσήλθαν στα εξωτερικά ιατρεία της ογκολογικής κλινικής του Πανεπιστημιακού Νοσοκομείου Ιωαννίνων από το Φεβρουάριο του 2008 μέχρι το Σεπτέμβριο του 2009. Aντίθετα για να εκτιμήσουμε τη σχετική με την υγεία ποιότητα ζωής των ασθενών με καρκίνο του μαστού και να εξερευνήσουμε το πώς αυτή συσχετίζεται με τους μηχανισμούς άμυνας, την ψυχική καταπόνηση και τον επιθυμητό βαθμό συμμετοχής στη λήψη θεραπευτικών αποφάσεων,αποφασίσαμε να επικεντρωθούμε σε ένα υπο- δείγμα ασθενών με μη μεταστατικό καρκίνο τουμαστού και να υιοθετήσουμε προοπτικό σχεδιασμό μελέτης. Συγχρονική μελέτη. Στόχοι: στόχος της συγχρονικής μελέτης ήταν η εκτίμηση των αναγκών πληροφόρησης και του επιθυμητού βαθμού συμμετοχής στη λήψη αποφάσεων των Ελληνίδων ασθενών με καρκίνο του μαστού. Μέθοδος: Η ελληνική εκδοχή της ‘Control Preference Scale’, ενός εργαλείου σχεδιασμένου για την εκτίμηση του επιθυμητού βαθμού συμμετοχής στη λήψη αποφάσεων, χορηγήθηκε σε 329 ασθενείς με καρκίνο του μαστού. Οι ανάγκες πληροφόρησης εκτιμήθηκαν με το Cassileth’s Information Styles Questionnaire, προσαρμοσμένο και αυτό στα ελληνικά. Η μέση ηλικία ήταν 59.5 (SD, 10.9) έτη. Ο μέσος χρόνος διάρκειας της νόσου ήταν 43.37 (SD, 53.7) μήνες. Η πλειοψηφία των ασθενών (53.5%) είχε καρκίνο του μαστού σταδίου ΙΙ κατά τη διάγνωσή τους και η πιο συχνή θεραπευτική αντιμετώπιση ήταν η ολική μαστεκτομή. Αποτελέσματα: Η πλειοψηφία των ασθενών (71.1%) προτιμούσαν παθητικούς ρόλους στη λήψη αποφάσεων με την πλειοψηφία αυτών να επιθυμούν να αφήσουν την ευθύνη της απόφασης εξολοκλήρου στο/η γιατρό τους (45.3%). Ο συνεργατικός ρόλος επιλέχτηκε από το 24% των ασθενών ενώ ο ενεργός από μόλις το 4.6%. Οι περισσότερες γυναίκες εξέφρασαν μία γενική επιθυμία για όσο το δυνατόν περισσότερη πληροφόρηση για τη νόσο τους (62.6%), αλλά ένα σημαντικό ποσοστό (37.4%) δεν επιθυμούσαν λεπτομερή πληροφόρηση, αντίθετα προτιμούσαν να μη γνωρίζουν δυσάρεστες πληροφορίες. Οι γυναίκες που επιθυμούσανλιγότερη πληροφόρηση και παθητικούς ρόλους ήταν πιο πιθανό να μην έχουν προσέλθει γιαμαστογραφία (p<0.001) ή και τεστ Παπανιοκολάου (p<0.0005) πριν διαγνωστούν με καρκίνοτου μαστού. Προοπτική μελέτη. Στόχοι: στοχεύσαμε στην εκτίμηση των ψυχολογικών μεταβλητών συσχετίζονται με τον επιθυμητό βαθμό συμμετοχής στη λήψη θεραπευτικών αποφάσεων και τη σχετική με την υγεία ποιότητα ζωής στις γυναίκες με μη- μεταστατικό καρκίνο του μαστού. Μέθοδος: 124 γυναίκες με μη-μεταστατικό καρκίνο του μαστού συμπλήρωσαν ερωτηματολόγια για το άγχος (Spielberger’s State–Trait Anxiety Inventory), την καταθλιπτική συμτωματολογία (Center for Epidemiologic Studies-Depression (CES-D), την ποιότητα ζωής (WHOQOL-BREF), και τους μηχανισμούς άμυνας του Εγώ (Life Style Index). Από αυτές, 82 (66.1%) συμμετείχαν στην επανεκτίμηση μετά από ένα χρόνο. Προκειμένου να εξετάσουμε το αν οι ψυχολογικές παράμετροι μπορούσαν να δρουν ως προγνωστικοί παράγοντες ή τροποποιητές υιοθετήσαμε μοντέλα πολλαπλής και ιεραρχικής παλινδρόμησης. Αποτελέσματα: Η καταθλιπτική συμπωματολογία και η σχετική με τη σωματική υγεία ποιότητα ζωής βελτιώθηκαν σημαντικά μέσα στο χρόνο. Το state άγχος, η σχετική με την ψυχική υγεία ποιότητα ζωής και η σχετική με το περιβάλλον ποιότητα ζωής παρέμειναν σταθερές. Η σχετική με τις κοινωνικές σχέσεις ποιότητα ζωής επιδεινώθηκε μέσα σε ένα χρόνο. Η μεγαλύτερη ηλικία (p = 0.021) και υψηλότερες τιμές στον μηχανισμό άμυνας της απώθησης (p = 0.044) συσχετίστηκαν ανεξάρτητα με επιθυμία για παθητικό ρόλο στη διαδικασία λήψης θεραπευτικών αποφάσεων. Το πρωιμότερο στάδιο καρκίνου (p = 0.043), τα χαμηλότερα επίπεδα state άγχους (p = 0.039), το χαμηλότερο σκορ απώθησης (p = 0.021) και η βελτίωση στην καταθλιπτική συμπτωματολογία (p < 0.001) απετέλεσαν προγνωστικούςπαράγοντες βελτίωσης στη σχετική με τη σωματική υγεία ποιότητα ζωής. Η ανάλυση τροποποιητή (moderation analysis) έδειξε ότι η επιθυμία για ενεργή συμμετοχή στη λήψη αποφάσεων προέβλεψε τη βελτίωση στη σχετική με τη σωματική υγεία ποιότητα ζωής αλλά μόνο στις γυναίκες με χαμηλότερα επίπεδα απώθησης. Συμπεράσματα. Τα αποτελέσματα της συγχρονικής μελέτης έδειξαν ότι το ποσοστό ασθενών που επιθυμούσαν παθητικούς ρόλους στη λήψη αποφάσεων ήταν το πιο υψηλό συγκρινόμενο με τα αντίστοιχα ποσοστά από παρόμοιες μελέτες σε άλλες χώρες. Αυτό ίσως να οφείλεται στην επίδραση του πατερναλιστικού προτύπου σχέσης γιατρού- ασθενούς στο ελληνικό σύστημα υγείας.
