Central venous oxygen saturation as new predictor of weaning from mechanical ventilation

Weaning patients from mechanical ventilation are one of the key elements in the course of the treatment of critically ill patients and could represent up to 40% of the time that a patient spends on mechanical ventilation. A prolongation of mechanical weaning is associated with increased risk of complications, such as ventilator-associated pneumonia, tracheal injury, bleeding from digestive stress ulcers and sepsis, while early extubation and reintubation could result to nosocomial pneumonia and increased mortality. Thus, identifying the optimal point when a patient can sustain spontaneous breathing is essential, but could be challenging. To the author’s knowledge, the data on the clinical significance of ScvO2 as a potential predictor of the SBT outcome is currently scarce. Thus, we conducted a prospective study aiming to evaluate whether ScvO2 values and their changes between the initiation and the end of a thirty-minute long SBT could independently predict the outcome of this trial, in a general surgical and medical population of mechanically ventilated patients. This is a prospective nested case-control study. All patients who were under mechanical ventilation for at least 48 hours in two medical Intensive Care Units (ICU) of “G. Papanikolaou” hospital between January 2011 and June 2013, were eligible for the study. Patients who were intubated for less than 48 hours, who had undergone an early tracheostomy (in less than 48 hours), who died before any weaning trials from mechanical ventilation were attempted and/or who did not have a central venous catheter, were excluded from final analysis. The rest constituted the final study population.A total of 77 patients (62.3% male) constituted the study population. The main diagnosis of ICU entry were infections (such as pneumonia or abdominal infections) (32.5%), major surgery (chest, abdominal, brain, vascular or orthopedic surgery) (39%), cardiac arrest (3.9%), trauma (9.1%), acute respiratory failure (2.5%), status epilepticus (3.9%), burns (1.3%), stroke (3.9%) and intoxication (3.9%). All patients underwent a 30-minute long SBT before an attempt for extubation was undertaken and based on the outcome of the trial were then separated in two groups: the group of patients with successful SBT and the group of patients with SBT failure. Overall, 49 patients (63.6%) had a successful SBT, with the rest 28 (36.4%) having an SBT failure. The SBT was considered as a success or failure at 30 minutes, based on the Task Force criteria, that is both subjective indices and objective measurements. Failure criteria of SBT included: 1) agitation and anxiety, altered mental status, dyspnea, cyanosis and clinical evidence of increased respiratory effort, 2) hypoxemia and/or hypercapnia 3) ineffective respiratory function and 4) hemodynamic complications. 49 patients who fulfilled the criteria of SBT success were then extubated. The remaining 28 patients had to return to ventilatory support by the end of the SBT because of clinical deterioration.Statistical analysis was performed using the Predictive Analytics Software (PASW) Version 18. A level of p<0.05 was considered significant to identify independent predictors of SBT success. All variables which were found to be univariately associated with a successful SBT outcome were then entered into a multivariate stepwise binary logistic regression analysis model, in order independent predictors of SBT success to be identified. Odds Ratios (OR) with corresponding 95% Confidence Intervals (CI) were reported for each predictor. Moreover, a Receiver Operating Characteristic (ROC) Curve was constructed for each SBT predictor, and the area under the curve was used as a visual index of the diagnostic accuracy of each one of them. Our study confirmed that ScvO2end as a single index was independently associated with a positive outcome and although its diagnostic accuracy was moderate (68.5% for ScvO2 and 71.5% for ΔScvO2), it was slightly superior to that of SaO2, which is a routinely screened parameter. ScvO2end predicted SBT success (OR:1.122; 95%CI:1.005-1.257) independently of age, hemoglobin concentration and arterial oxygen saturation (SaO2end). A decrease of less than 4% in ScvO2 values between the beginning and the end of an SBT was also independently associated with a successful outcome (OR=18.278; 95%CI=4.017-83.163, p<0.001), along with age, Hb concentration and SaO2end. In this prospective nested case-control study conducted in a general ICU population, a ScvO2 drop of at least 4% was independently associated with failure to wean from mechanical ventilation. Judging whether the SBT is successful or not, in order to proceed to extubation, is depended not on a single index but on a combination of several objectively measured and clinically evaluated parameters. Whether adding the routine evaluation of ScvO2 in this algorithm might improve the ability to correctly identify those patients who could safely proceed to extubation or not, remains to be tested in large prospective trials in the future.Η αποδέσμευση από τον μηχανικό αερισμό (weaning) αποτελεί ακρογωνιαίο λίθο της αντιμετώπισης των βαρέως πασχόντων ασθενών στη ΜΕΘ και μπορεί να αντιπροσωπεύει έως το 40% του συνολικού χρόνου μηχανικής υποστήριξης της αναπνοής. Από τη μία πλευρά, η καθυστέρηση της έναρξης του weaning μπορεί να επιδεινώσει την ατροφία των αναπνευστικών μυών και να αυξήσει την πιθανότητα επιπλοκών από την παρατεταμένη διασωλήνωση της τραχείας, τον κίνδυνο νοσοκομειακής πνευμονίας και σήψης, με τελικό αποτέλεσμα την αύξηση της θνητότητας. Από την άλλη πλευρά όμως, η πρώιμη αποδέσμευση από τον αναπνευστήρα, όταν ο άρρωστος δεν είναι ακόμη έτοιμος να αναπνέει μόνος του, μπορεί να έχει αρνητικές ψυχολογικές αλλά και παθοφυσιολογικές επιπτώσεις. Η βελτίωση και αντιστροφή των βλαβών, που αποτέλεσαν την ένδειξη για διασωλήνωση και μηχανικό αερισμό, σηματοδοτεί τον ιδανικό χρόνο έναρξης του weaning. H προγνωστική σημασία του κορεσμού οξυγόνου αίματος από κεντρικό φλεβικό καθετήρα (ScvO2), ως πιθανού δείκτη αποδέσμευσης από τον μηχανικό αερισμό, υπήρξε έως σήμερα ασαφής και μη επαρκώς αποδεδειγμένη. Για το σκοπό αυτό, εκτελέσαμε μετρήσεις σε ασθενείς σε μηχανική υποστήριξη της αναπνοής, οι οποίοι απέτυχαν σε μια δοκιμή αυτόματης αναπνοής διάρκειας 30 λεπτών (cases). Παράλληλα έγιναν μετρήσεις σε μια ομάδα ελέγχου (μαρτύρων), που ανταποκρίθηκαν επιτυχώς στη δοκιμή και ως εκ τούτου αποσωληνώθηκαν. Πιο συγκεκριμένα, προσδιορίστηκαν οι τιμές του κορεσμού οξυγόνου (ScvO2), αμέσως πριν την έναρξη και στο τέλος της δοκιμής αυτόματης αναπνοής. Η μελέτη ολοκληρώθηκε στη διάρκεια μιας χρονικής περιόδου 2.5 ετών (Iανουάριος 2011 - Ιούνιος 2013), σε δύο πολυδύναμες μονάδες εντατικής θεραπείας (medical-surgical ICU) του νοσοκομείου Γ. Παπανικολάου (Θεσσαλονίκη). Στα κριτήρια ένταξης της μελέτης ανήκαν ασθενείς διασωληνωμένοι τουλάχιστον για 48 ώρες, λόγω οξείας αναπνευστικής ανεπάρκειας ή υποβολής σε μείζονες χειρουργικές επεμβάσεις. Ασθενείς διασωληνωμένοι λιγότερο από 48 ώρες, ή ασθενείς που είχαν υποβληθεί σε τραχειοστομία, ή απεβίωσαν πρόωρα, καθώς και ασθενείς χωρίς τοποθετημένο κεντρικό φλεβικό καθετήρα αποκλείστηκαν από τη μελέτη. Συνολικά 77 ασθενείς με ηλικιακό εύρος 18-86 ετών αποτέλεσαν τον πληθυσμό της μελέτης (48 άνδρες - 62.3%). H αιτία εισαγωγής στη ΜΕΘ ήταν: λοίμωξη (32.46%), μετεγχειρητική παρακολούθηση (38.96%), καρδιακή ανακοπή (3.89%), τραύμα-ΚΕΚ (9.09%), φαρμακευτική δηλητηρίαση (3.89%), έγκαυμα (1.29%), ΑΕΕ (3.89%), κρίση επιληψίας (3.89%), πνευμοθώρακας με οξεία αναπνευστική ανεπάρκεια (2.59%).H τελική αξιολόγηση στηρίχτηκε τόσο σε υποκειμενικά κριτήρια όσο και αντικειμενικές μετρήσεις στο τέλος της δοκιμής (οδηγίες International Consensus Conference 2007). Κριτήρια αποτυχίας της δοκιμής ήταν: 1) άγχος και ανησυχία, μεταβολή του επιπέδου συνείδησης, κυάνωση ή ενδείξεις αυξημένου έργου αναπνοής, 2) υποξαιμία/υπερκαπνία, 3) ελαττωμένη πνευμονική λειτουργία και 4) αιμοδυναμικές επιπλοκές. 49 ασθενείς (63.6%) πέτυχαν στη δοκιμή αυτόματης αναπνοής και ως εκ τούτου αποσωληνώθηκαν. Αντίθετα, οι υπόλοιποι 28 (36.4%) επανήλθαν σε μηχανική υποστήριξη της αναπνοής, λόγω επιδείνωσης της κλινικής τους κατάστασης. Για τη βασική στατιστική ανάλυση χρησιμοποιήθηκε το στατιστικό πακέτο Predictive Analytics Software (PASW) έκδοση 18. Το επίπεδο σημαντικότητας της μελέτης ορίστηκε σε p<0.05 με σκοπό τη διερεύνηση των ανεξάρτητων προγνωστικών δεικτών έκβασης της δοκιμής αυτόματης αναπνοής. Όλες οι μεταβλητές, οι οποίες στην μονοπαραγοντική ανάλυση βρέθηκε να σχετίζονται με επιτυχία της δοκιμής, χρησιμοποιήθηκαν στη συνέχεια σε πολυπαραγοντικό μοντέλο λογαριθμιστικής εξάρτησης, ώστε να προσδιοριστούν οι ανεξάρτητοι προγνωστικοί δείκτες επιτυχίας της δοκιμής. Για κάθε δείκτη ξεχωριστά υπολογίστηκαν ο λόγος πιθανοτήτων (odds ratio) με τα 95% διαστήματα εμπιστοσύνης εκατέρωθεν της μέσης τιμής και σχεδιάστηκαν οι καμπύλες ROC, ώστε το εμβαδόν κάτωθεν της καμπύλης να αποτυπώνει τη διαγνωστική τους ακρίβεια (diagnostic accuracy). Η μελέτη μας επιβεβαίωσε ότι η τιμή του ScvO2 στο τέλος της δοκιμής (ScvO2end) αποτελεί ανεξάρτητο δείκτη πρόβλεψης της επιτυχούς έκβασης, με διαγνωστική ακρίβεια 68.5% για ScvO2 και 71.5% για ΔScvO2, ανώτερη δηλαδή από αυτήν του SaO2, της πιο χρήσιμης και ευρύτερα υπολογιζόμενης έως σήμερα παραμέτρου. O ScvO2end συσχετίστηκε με επιτυχία της δοκιμής (OR:1.122; 95%CI:1.005-1.257) ανεξάρτητα από την ηλικία, την αιμοσφαιρίνη (Hb) και τον κορεσμό οξυγόνου του αρτηριακού αίματος SaO2end.
