Placebo analgesia is a manipulation of cognitive expectancy whereby the subject's expectation of pain relief promotes diminished pain ratings. Endogenous opioids have been found to play a role in mediating this phenomenon, and advances in functional neuroimaging have allowed the in vivo examination of pain- and placebo-induced opioid neurotransmission in humans. The brain circuitry activated during placebo analgesia exhibits substantial overlap with that involved in motivation, suggesting that similar cognitive elements might be involved in the placebo effect.
