A Randomized Comparative study of Recovery Characteristics and Hemodynamic Parameters of Desflurane and Sevoflurane in Functional Endoscopic Sinus Surgery

BACKGROUND: Modern inhaled anaesthetics, Sevoflurane and Desflurane with their low blood-gas partition coefficients facilitate rapid induction and prompt recovery from anaesthesia. In this study, we compared the recovery characteristics and hemodynamic parameters of Desflurane and Sevoflurane in functional endoscopic sinus surgery. OBJECTIVE: The objective was to assess the Intra-operative hemodynamic parameters (HR and MAP) and emergence and recovery characteristics of Desflurane in comparison to Sevoflurane. METHODS AND MATERIALS: A total of 100 patients under the age group of 18-65yrs belonging to ASA PS I and II undergoing functional endoscopic sinus surgery were enrolled for this study. They were randomly assigned into two groups of 50 each in Desflurane (Group D) and Sevoflurane (Group S). After standardized induction sequence with Thiopentone and Vecuronium, patient either received Desflurane with a end tidal concentration of 3% or Sevoflurane with an end tidal concentration of 1% combined with 66% of Nitrous oxide and oxygen mixture for maintenance of anaesthesia. The hemodynamic parameters HR and MAP were measured at every 15 mins interval till the end of surgery. At the end of the procedure, the time of discontinuation of anaesthetic agents was noted as time zero and from then on all subsequent recovery times- spontaneous movement, response to painful stimuli, extubation, recall of name, hand grip, limb lift on command were determined at 1-min intervals till a PARS > 10. RESULTS: The demographic profile and intraoperative heart rate were comparable in both groups. However, fall in mean arterial pressure was significant in sevoflurane group with p value of 10 was significantly higher in patients in the desflurane group. No significant nausea and vomiting was noted. CONCLUSION: Desflurane used for maintenance of anaesthesia offered a stable hemodynamic status and earlier recovery with no significant postoperative nausea and vomiting in comparison to Sevoflurane
