Discrimination in Health Services by university students in a city in southern Brazil

This study is nested to a master consortium- 2017/2018 of the Graduate Program in Epidemiology of the Federal University of Pelotas. 1865 students answered a self-administered and anonymous questionnaire, over 18 years of age, enrolled at the Federal University of Pelotas in the first semester of 2017 and who were attending the second semester of one the 80 courses participating in this research. Data were collect with the goal of evaluating the discrimination in health services suffered in the students’ lives. The reasons for perceived discrimination in the health services questioned were: Lack of money, Social class, Race/skin color, Type of occupation; Type of disease; Sexual preference; Religion/belief; Sex, Age and others. The service nature where the discrimination occurred were differentiate between SUS, private or health insurance. The professional linked to the outcome was categorized as a receptionist or administrator; security of service; nurse technician, nurse, doctor, dentist and others. The association with the outcome was evaluate according to the sociodemographic variable that identified the economic class according to the Brazilian Association of Companies and Research (ABEP). The demographic variables for the association were sex, age, skin color, region of origin of the student. In addition, the behavioral variable characterized the students in smokers, ex-smokers and nonsmokers. The description of the prevalence of each reason for perceived discrimination was calculate with a 95% confidence interval. Total discrimination was calculate by summing up all grounds for discrimination. In addition, the associations were calculate by crude and adjusted with Poisson regression. The main results were a total prevalence of discrimination in the health services throughout the life of 22.8%, and the most related reasons were social class and lack of money, 11% (95% CI 9.0, 12, 0) and 10.1% (95% CI 9.0, 12.0). The SUS was service nature where most discrimination occurred (58%). The doctors (51.1%) and the receptionists (42.4%) were the professionals most associated with discrimination. Pelotas (59.1%) was refer to as the site of greatest discrimination occurrence. In summary, the prevalence observed was one of the highest in the literature, which indicates the magnitude of the problem of the professional-patient relationship. Um questionário auto aplicado e anônimo foi respondido por 1865 alunos, maiores de 18 anos, ingressantes da Universidade Federal de Pelotas no primeiro semestre de 2017 e que estavam cursando o segundo semestre de um dos 80 cursos participantes desta pesquisa. Foram coletados dados com o objetivo de avaliar a discriminação nos serviços de saúde sofrida alguma vez na vida dos estudantes. Os motivos da percepção de discriminação nos serviços de saúde questionados foram: Falta de dinheiro, Classe social, Raça/cor; Tipo de ocupação; Tipo de doença; Preferência Sexual; Religião/ crença; Sexo; Idade, e Outros. A natureza do serviço onde ocorreu a discriminação foram diferenciados entre SUS, particular ou plano de saúde. O profissional vinculado ao desfecho foi categorizado em recepcionista ou administrador; segurança do serviço; técnico de enfermagem, enfermeiro, médico, dentista e outro. A associação com o desfecho foi avaliada segundo a variável sociodemográfica que identificou a classe econômica de acordo com a Associação Brasileira de Empresas e Pesquisas (ABEP). As variáveis demográficas para a associação foram sexo, idade, cor da pele, região de procedência do aluno. E a variável comportamental caracterizou os alunos em fumantes, ex-fumantes e não fumantes. A descrição da prevalência de cada motivo de discriminação percebido foi calculada com intervalo de confiança de 95%. A discriminação total foi calculada por meio do somatório de todos os motivos de discriminação. E as associações foram calculadas por análises bruta e ajustada com regressão de Poisson. Os principais resultados foram uma prevalência total de discriminação nos serviços de saúde ao longo da vida de 22,8%, sendo que os motivos mais relacionados foram a classe social e a falta de dinheiro, 11% (IC95% 9,0;12,0) e 10,1% (IC95%9,0;12,0). O SUS foi o serviço de saúde onde mais ocorreram as discriminações (58%). Os médicos (51,1%) e os recepcionistas (42,4%), foram os profissionais mais vinculados com a discriminação. Pelotas (59,1%) foi referida como o local de maior ocorrência da discriminação. Em síntese, a prevalência observada foi uma das maiores da literatura, o que indica a magnitude do problema da relação profissional-paciente. Neste volume constam: o projeto de pesquisa, o relatório de trabalho de campo, as alterações realizadas no projeto original, a nota para a imprensa e os apêndices e anexos, os quais incluem o protocolo de aceite do Comitê de Ética em Pesquisa, o termo de consentimento livre e esclarecido do estudo, o questionário utilizado na coleta de dados, o manual de instruções e as normas de publicação do Caderno de Saúde Pública, para o qual será submetido o artigo desta dissertação
