Adverse effects with the use of immunological control point inhibitors

La terapia de bloqueo del punto de control inmunitario difiere de las terapias tradicionales en su mecanismo de acción, respuesta y perfil de efectos adversos. Las Palabras claves usadas fueron: inhibidores de punto de control, muerte programada, ligando PD-L1, CTLA-4, PD-1, agente antineoplásico, inmunológico, efecto adverso, evento adverso. El objetivo general fue describir los efectos adversos relacionados con el uso de inhibidores de punto de control inmunitario en cáncer según la clasificación de los criterios comunes de terminología para eventos adversos. Se realizó una revisión sistemática de la literatura de estudios en pacientes en tratamiento con agentes anti PD-1, PD-L1 y CTLA4, que informaron efectos adversos con el uso de este tipo de terapia. Se incluyeron 54 artículos los cuales se analizaron mediante un instrumento de lectura crítica y, con la información extraída, se diligenciaron las matrices para su posterior análisis, de estos artículos 27 corresponden a estudios integrativos y 27 a estudios primarios. Los efectos adversos más reportados fueron: prurito, erupciones maculopapulares, colitis, neumonitis, hipertiroidismo y fatiga, los cuales son descritos como poco frecuentes en comparación con otras terapias, pero pueden ser potencialmente mortales. En consecuencia, la educación con respecto a estos perfiles de toxicidad a pacientes, familia, cuidadores y personal de salud en atención primaria, es de importancia para el éxito de la terapia. Finalmente, se pudo concluir que la inmunoterapia es una nueva estrategia en el tratamiento del cáncer, la cual no es ajena a la presencia de efectos adversos y debido a la novedad de su utilización es necesario un seguimiento en el tiempo con respecto a su perfil de toxicidad.Immune checkpoint block therapy differs from traditional therapies in its mechanism of action, response and adverse effects profile. The keywords used were: control point inhibitors, programmed death, ligand PD-L1, CTLA-4, PD- 1, antineoplastic agent, immunological, adverse effect, adverse event. The general objective was to describe the adverse effects related to the use of immune control point inhibitors in cancer according to the classification of common terminology criteria for adverse events. A systematic review of the study literature was conducted in patients treated with anti-PD-1, PD- L1 and CTLA4 agents, who reported adverse effects with the use of this type of therapy. 54 articles were included, which were analyzed using a critical reading instrument and matrices were filled out with the information extracted for later analysis. 27 articles corresponding to integrative studies and 27 to primary studies. The most reported adverse effects were: pruritus, maculopapular rashes, colitis, pneumonitis, hyperthyroidism and fatigue. They are rare compared to other therapies, but they can be life threatening. The education to patients, family, caregivers and health personnel in primary care, about this toxicity profile, is of importance for the success of the therapy. It could be concluded that immunotherapy is a new strategy in the treatment of cancer.
