Improving remote ultrasound examinations – the ASYSTED system provides expert knowledge and teleguidance

Objectives Ultrasound examinations are usually performed by physicians. Especially in isolated locations like rigs and on vessels where there are no physicians present patients have to be evacuated in cases where an ultrasound examination is needed. This is often associated with life-threatening delays and significant transport costs. ASYSTED (Advanced System for Tele-Guided Diagnostics) has been developed to enable trained non-physicians to perform ultrasound examinations supported by remote experts. The examiner is guided using a telemedicine system which transmits audio and visual instructions to handle the ultrasound probe. Standard DICOM (Digital Imaging and Communications in Medicine) images sent to the expert allow an immediate diagnosis. Methods The patented system ASYSTED provides an intuitive and cost-effective method for remote examiner guidance and expert consultation. The system is a software and hardware combination supporting any IP connections, especially over satellite. Thus it can be used ubiquitously (water, land, air) together with any ultrasound system providing DICOM imaging and a video output. From a prototype at present ASYSTED is being developed to a marketable system with our partner for telemedicine systems. A system evaluation in early autumn is in preparation. Results By using military proven videoconference systems a system for teleguided ultrasound examinations and remote diagnostics will be available for the market in 2016. ASYSTED can be used on vessels or off-shore rigs as well as in mobile scenarios (SAR) and remote regions. The final system will be intuitive and can be upgraded for further medical and non-medical purposes (multiple use). Connections with standard radiological information systems such as RIS/PACS (Picture Archiving and Communication System) or HIS (hospital information systems) are possible. Conclusion Any ultrasound system providing DICOM imaging and transfer and a video output can be upgraded with ASYSTED. Expensive evacuation costs can be avoided and access to qualified medical care can be accelerated
