The scale-up of antiretroviral treatment (ART) of HIV/AIDS in resource-limited settings has been one of the largest public health operations of our time, and by the end of 2009 more than 5 million people were receiving ART in low- and middle-income countries. Although several studies on ART efficacy in Africa have been published, the majority have been carried out in larger cities and usually with short follow-up time. To date, there is a paucity of research from rural settings, which often face additional challenges, such as shortages of health workers, transport difficulties and other logistical constraints.
