Outcomes for older people with chronic and complex needs: a longitudinal examination of the use of community services following an aged care assessment in Victoria

Researchers examined the community services recommended for older people with complex needs, the extent to which such recommendations were taken up and the ACAS clients’ and carers’ experience of services. They found that difficulties in accessing community services were related to social isolation of the older person, high carer burden and the older person reporting low mood. Conversely the community care system is working best for older people who have someone (such as a family member or case manager) to help them to access the required services. Recommendations are made for improved identification of, and support for, those at risk of not accessing recommended services
