Meanings of chronic musculoskeletal pain: a psychoanalytic psychosomatic perspective

Objective: To know and analyze the meanings involved in the life history of people with chronic musculoskeletal pain from the perspective of the psychoanalytic psychosomatic. Method: qualitative research, anchored by the theoretical reference of the psychoanalytic psychosomatic. The study was attended by 20 people who presented chronic muscular pain and who were treated with physical therapy in a clinic in a city in the interior of the State of São Paulo, as they were invited to talk about their pains and their lives. Data collection was performed through semi-structured interviews and analyzed through thematic content analysis. Results and Discussion: The participants' ages ranged from 18 to 82 years, the majority of them female and gonarthrosis was the most prevalent disease. Most of the participants presented pain for more than 5 years. The analysis of the material resulted in two themes "Life under pain, life difficult and "My life, my body is a hematoma, a chronic pain". The first theme reflected the life of existences and lives in the reports of actions for patients and work for lived in many cases patients in life processes. In the second theme, there are opportunities that are associated with the reporting of intense sufferings. This theme was deployed in two sub-themes: "Emotional-affective suffering" and "After a feeling, a pain comes". The first sub-theme was related to experiences of illness and / or loss of family members, situations of intrafamily violence, abandonment, relationships denoting lack or absence of adverse effects, in addition to conflicting relationships between family members. The second sub-theme covers the explicit accounts in which such experiences occasioned physical pain. In general, the results permeate an important issue for the psychosomatic - the link between biography and pain. There have been reports of events in their lives, of significant experiences in which the participants more or less consciously associated these experiences with their chronic musculoskeletal pain, since such pains appeared almost simultaneously, only some time later or were associated with some significant event . Conclusion: From the perspective of the psychoanalytic psychosomatics, several meanings of chronic musculoskeletal pain were identified, from violence, threats, resentments, illnesses, deaths, guilts and marital separations. From what was presented in this study it is important to consider the importance and necessity of investigating the subjectivity, biography and experience of people living with chronic musculoskeletal pain as part of the professional conducts for the treatment and management of pain, even though instruments to attest to the presence of alexithymia or to understand that the explanations of somatizations are in the failure of representation.Não recebi financiamentoObjetivo: Conhecer e analisar os significados envolvidos na história de vida de pessoas com dor musculoesquelética crônica pela perspectiva da psicossomática psicanalítica. Método: pesquisa qualitativa, ancorada pelo referencial teórico da psicossomática psicanalítica. Participaram do estudo 20 pessoas apresentando dores musculoesqueléticas crônicas e que estavam em tratamento com fisioterapia em uma clínica escola de um município do interior de São Paulo as quais foram convidadas a falar sobre suas dores e sua vida. A coleta dos dados foi realizada através de entrevistas semiestruturadas que foram analisadas através de análise de conteúdo temática. Resultados e discussão: A idade dos participantes variou entre 18 e 82 anos, sendo a maioria do sexo feminino e a gonartrose foi a doença mais prevalente. A maioria dos participantes apresentavam dor há mais de 5 anos. Da análise do material resultaram dois temas “Vida sofrida, vida difícil” e “Minha vida, meu corpo é um hematoma, uma dor crônica”. O primeiro tema reflete a vida sofrida e difícil nos relatos de situações de esforços físicos e trabalhos pesados vivenciados desde a infância que em alguns casos continuaram em outras fases da vida. No segundo tema, estão incluídas experiências que foram associadas ao relato de sofrimentos intensos. Esse tema foi desdobrado em dois subtemas: “Sofrimentos afetivo-emocionais” e “Depois de um sentimento, vem uma dor”. O primeiro subtema foi relacionado a vivências de doenças e/ou perdas de pessoas da família, situações de violência intrafamiliar, abandonos, relações denotando insuficiência ou até ausência de vínculo afetivo, além das relações conflituosas entre membros da família. O segundo subtema abarca os relatos explícitos em que tais experiências ocasionaram a dor física. De maneira geral, os resultados permeiam uma questão importante para a psicossomática – a vinculação entre biografia e dor. Surgiram relatos sobre fatos das suas vidas, de vivências significativas em que os participantes associaram de maneira mais ou menos consciente estas vivências às suas dores musculoesqueléticas crônicas, já que tais dores surgiram quase que simultaneamente, apenas algum tempo depois ou foram associadas a algum evento significativo. Conclusão: pelo referencial da psicossomática psicanalítica vários significados das dores musculoesqueléticas crônicas foram identificados, desde violências, ameaças, ressentimentos, doenças, mortes, culpas e separações conjugais. A partir do que foi apresentado nesse estudo é importante considerar a importância e a necessidade de se investigar a subjetividade, a biografia e a experiência de pessoas que vivem com dor musculoesquelética crônica como parte das condutas profissionais para o tratamento e manejo da dor, mesmo que instrumentos venham atestar a presença de alexitimia ou que se entenda que as explicações das somatizações estejam nas falhas de representação
