Efeito da colocação da órtese Pediasuit na oscilação postural durante a atividade de alcance manual na postura sentada em crianças com paralisia cerebral

Children with cerebral palsy (CP) have limitations in the components of the structure and function of the body, social participation and implementation of activities, being one of these activities manual reaching. From this evidence was our interest in the Study I, a systematic review aimed to gather studies evaluating the scope for CP children to identify the variables analyzed review and discuss their findings. It was found that children with CP have limitations in reaching, characterized by lower smoothness and linearity of movement. Among the causes of these limitations are the postural control deficits. Postural control refers to the ability to control the body in relation to space, when it is disturbed during dynamic activities such as in the range of objects. Anticipatory postural adjustments (APA) and compensatory (APC) are generated to keep the balance forward to this destabilization. There are few studies evaluating the APA and APC in CP children with different impairments of gross motor function and manual function during manual range in the seated position. Therefore, we developed the Study 2 aimed to check the APA and APC during manual reaching of PC children with mild and moderate-severe impairment of motor function and manual-thick comparing them with typical developmentTD children. It was found that children with CP with mild and moderate-severe impairment exhibit greater instability in the APA and APC when comparads DT, however not present disparity when compared with children with different impairments of gross motor function and manual. After this evidence that children with CP have instability in the APA and APC there is a need for studies that verify therapeutic strategies, as well as auxiliary devices aimed aprimor postural control of children with CP during the reaching in the seated position. Thus originated the Study 3 aimed to determine the immediate effect of suit Pediasuit in APA and APC during manual reaching in sitting posture in children with spastic CP. It was found that the suit Pediasuit provides immediate changes in postural control during the sitting reaching of children with CP, providing greater stability of the center of pressure (COP) in the APA and APC. Thus, we can conclude that children with CP have deficits of range of motion and postural control during the activity of reach when compared to children with typical development, demonstrating postural instability during the activity. The suit Pediasuit generates greater stability in the APA and APC in children with CP. Therefore we recommend the use of suit in clinical practice in order to obtain immediate changes in postural stability.Financiadora de Estudos e ProjetosCrianças com Paralisia cerebral (PC) apresentam limitações nos componentes da estrutura e função do corpo, na participação social e na execução de atividades, sendo uma destas atividades o alcance manual. A partir dessa evidência houve o interesse em realizar o Estudo I, uma revisão sistemática que objetivou reunir estudos que avaliaram o alcance em crianças com PC para identificar as variáveis analisadas, rever e discutir suas descobertas. Constatouse que crianças com PC apresentam limitações no alcance, caracterizada por menor suavidade e linearidade do movimento. Dentre as causas destas limitações estão os déficits de controle postural. O controle postural refere-se à habilidade de controlar o corpo em relação ao espaço, quando este é perturbado, durante atividades dinâmicas como no alcance de objetos. Ajustes posturais antecipatórios (APA) e compensatórios (APC) são gerados para manter o equilíbrio frente à esta desestabilização. Há escassez de estudos que avaliaram o APA e APC em crianças com PC com diferentes comprometimentos da função motora grossa e função manual durante o alcance manual na postura sentada. Sendo assim, foi desenvolvido o Estudo II que objetivou verificar o APA e APC durante o alcance manual de crianças PC com comprometimento leve e moderado-grave da função motora-grossa e manual comparando-as com crianças desenvolvimento típico (DT). Constatou-se que crianças com PC com comprometimento leve e moderado-grave exibem maior instabilidade no APA e APC quando comparadas a DT, no entanto não apresentam disparidade quando comparadas com crianças com diferentes comprometimentos da função motora grossa e manual. Após essa evidência que crianças com PC apresentam instabilidade no APA e APC há a necessidade de estudos que verifiquem estratégias terapêuticas, bem como dispositivos auxiliares que visem aprimoramento do controle postural de crianças com PC durante o alcance na postura sentada. Assim teve origem o Estudo III que objetivou verificar o efeito imediato da órtese Pediasuit no APA e APC durante o alcance manual na postura sentada em crianças com PC espástica. Constatou-se que a órtese Pediasuit proporciona mudanças imediatas no controle postural sentado durante o alcance de crianças com PC, conferindo maior estabilidade do centro de pressão (COP) no APA e APC. Assim, pode-se concluir que as crianças com PC apresentam déficits dos movimentos de alcance e do controle postural durante a atividade do alcance quando comparado às crianças com desenvolvimento tipico, demonstrando instabilidade postural durante a atividade. A órtese Pediasuit gera maior estabilidade no APA e APC nas crianças com PC. Sendo assim, recomenda-se o uso da órtese na prática clínica, visando obter mudanças imediatas na estabilidade postural
