DESENTRALISASI DAN PENGANGGARAN OBAT PADA DINAS KESEHATAN KABUPATEN PROPINSI NUSA TENGGARA TIMUR (NTT

Background: Drugs play a very important role in managing and improving health status. The local income (PAD) and general allocation fund (DAU) that are limited and the views of local authority (PEMDA) and district house of common (DPRD) on health that is considered as a consumptive sector is the main problem in ensure the sustainability of drugs financing in time of decentralization. This study was aimed at finding out the budgeting and quality of drugs planning in the District Health Office in the era of decentralization. Methods: This was a descriptive explorative study using qualitative and quantitative data. The study was held in East Nusa Tenggara (NTT). The analysis units were District Health Office (DKK) chosen purposively, i.e. South Central Timor (TTS) representing DKK with P2M intensification projectsBelu and North Central Timor (TTU) as an area without projects. The predictor was decentralization and the criterion was drugs budgeting. The moderator variables were local budget and expenditure (APBD), DAU, PAD, local political commitment, and quality of drugs planning. Data were gathered by observing documents and interviews with the Chief of District Health Office. Results: The amount of local budget and expenditure (APBD) of TTS in 2001 increased 265%TTU 421%, and Belu 363%. The drugs budget in health district of TTS in 2001 decreased 8%, Belu decreased 67%, and TTU increased 80%. The percentage of drugs budget of the local budget and expenditure (APBD) decreased from 1.90-3.94% in the year of 2000 to 0.36-0.85% in 2001. In average the key drugs availability in all health districts after the decentralization in the condition of 25-39 months above secure level of 10-22 months.The inefficiency value as a result of over procurement of drugs in TTS reached 10%, TTU 7% and Belu 2% of the total budget for drugs. Conclusion: Drugs budget after decentralization is influenced more by the quality of planning and financial capability (DAU). The drugs planning in TTS is better in inputting data disease data and data access. In all three regencies, the availability of drugs is in average sufficient after the decentralization. Recommendation: Drugs over budget should be used in drugs acquisition, but it is allocated for improving drugs service quality (label, packages, written information).
