Relação da disfunção temporomandibular (DTM) e Síndrome de Eagle: revisão sistemática da literatura

Temporomandibular dysfunction (TMD) is a collective term used for problems associated with changes in the temporomandibular joint, chewing muscles, associated structures, or both. Some of its symptoms can be found similarly in Eagle syndrome, which is related to the elongation of the styloid process of the temporal bone or ligaments that are inserted therein. The aim of this study was to perform a systematic review of the literature on the relationship between TMD and Eagle syndrome. The databases of the PubMed, Virtual Health Library and LILACS were used with the terms: "Temporomandibular joint dysfunction syndrome" AND "Eagle syndrome", "Facial pain" AND "Eagle syndrome". The MeSH method of the PubMed database was used to find the synonymous terms, which were correlated with each other. The searches resulted in a total of 4,629 articles, which were submitted to title analysis and duplicate removal, leaving only 24 articles, which were analyzed according to the summary, applying the following inclusion criteria: a clinical study conducted in the use of the Diagnostic Criteria for Temporomandibular Disorders (RDC / TMD) or Diagnostic Criteria for Temporomandibular Disorders (DC / TMD) for the diagnosis of TMD and the panoramic and cephalometric radiographs or computed tomography (CT) for the diagnosis of Eagle syndrome. Twelve articles were selected for the complete reading of the text, however only three were part of the research. In no study was the relationship between said comorbidities, in addition to coincident symptoms. It was observed that although CT is considered the gold standard image for the diagnosis of Eagle syndrome, panoramic and cephalometric radiographs are adequate imaging, however it is of fundamental importance to perform a specific anamnesis and physical examination for investigation and diagnosis with TMD, thus directing appropriate therapy. It was concluded that the relationship between TMD and Eagle syndrome is associated with the presence of similar signs and symptoms, which may cause difficulty in the differential diagnosis and treatment behavior, but with the use of the diagnostic methods suggested in the literature, will provide easy perception and specification. The treatment plan is based on clinical and radiographic findings, with conservative treatment for TMD and surgical treatment for Eagle syndrome advocated.Disfunção temporomandibular (DTM) é um termo coletivo usado para problemas associados às alterações na articulação temporomandibular, músculos da mastigação, estruturas associadas ou ambas. Alguns de seus sintomas podem ser encontrados de forma similar na síndrome de Eagle, a qual está relacionada ao alongamento do processo estiloide do osso temporal ou ligamentos que nele se inserem. O objetivo deste trabalho foi realizar uma revisão sistemática da literatura sobre a relação entre a DTM e a síndrome de Eagle. Foram usadas as bases de dados do PubMed, Biblioteca Virtual em Saúde e LILACS com os termos: ‘‘Temporomandibular joint dysfunction syndrome” AND “Eagle syndrome”, “Facial pain” AND “Eagle syndrome”. O método MeSH da base PubMed foi usado para encontrar os termos sinônimos, os quais foram correlacionados entre si. As buscas resultaram em um total de 4.629 artigos, que foram submetidos a análise do título e remoção dos duplicados, restando apenas 24 artigos, os quais foram analisados de acordo com o resumo, aplicando-se os seguintes critérios de inclusão: estudo clínico realizado em seres humanos utilizando o Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) ou Diagnostic Criteria for Temporomandibular Disorders (DC/TMD) para diagnóstico da DTM e as radiografias panorâmica e cefalométrica ou tomografia computadorizada (TC) para diagnóstico da síndrome de Eagle. Foram selecionados 12 artigos para a leitura completa do texto, no entanto apenas três fizeram parte da pesquisa. Em nenhum estudo foi encontrada a relação entre as referidas comorbidades, além dos sintomas coincidentes. Foi observado que apesar da TC ser considerada o exame de imagem padão-ouro para o diagnóstico da síndrome de Eagle, as radiografias panorâmica e cefalométrica são exames de imagem adequados, porém é de fundamental importância realizar uma anamnse e exame físico específicos para investigação e diagnóstico diferencial com a DTM, direcionando assim a terapêutica adequada. Concluiu-se que a relação entre DTM e síndrome de Eagle está associada a presença de sinais e sintomas semelhantes, os quais podem causar dificuldade no diagnóstico diferencial e conduta de tratamento, porém com a utilização dos métodos diagnósticos sugeridos na literatura, proporcionará fácil percepção e especificação. O plano de tratamento é baseado nos achados clínicos e radiográficos, sendo preconizado o tratamento conservador para a DTM e cirúrgico para a síndrome de Eagle
