A atividade de trabalho e a saúde das enfermeiras na estratégia saúde da família- ESF

The Program for Family Health - PSF, implanted in Brazil more than one decade, it comes consolidating as strategy of reorientation of care practices for health care. It is observed, however, that its implantation and development in the Brazilian s cities, and part of the diverse involved actor s understanding (managers, professionals and population), has occurred of varied form. Our study intends to analyze the relation of the work s activity of the ESF nurse s in the I, III, IV and V health districts of João Pessoa, PB. We have as specific s objectives: to investigate the conditions and the organization of the work; to identify the regulate activities of nurses, developed for them; to analyze the experiences of suffering and pleasure and the entailed processes of illness to this type of activity. We appeal in port to them theoretical of the ergonomics of the activity, that provides conceptual tools to apprehend the work s situations and the operative ways, developed by the nurses in its labor action, and their strategies to regulate of the variability, due of the imbalance between the prescribed work and the work effectively carried through; and the psychodinamics of work , that if leans over on the dynamic analysis of the intersubjective processes lived deeply in the situations of work and its relations with the health of that they work. Of the metodologic s point of view, half-structuralized twelve interviews had been used, aiming at to get the maximum amplitude in the description, understanding and explanation of the focus of the study, and four comments of the activity of these professionals from different health districts. The work consists of twelve interviews with nurses of Family Health Units - USFs, lócus of the program, that they had accepted to participate voluntarily of the research. The analysis of the produced data was the thematic analysis of content. Preliminarily was percieved in the Family Health Units, the nurses develops multiple activities in the field of the assistance, the management and the education/formation, what she extends its responsibility. Of this form, the assumed actions, such are innumerable as: to be able to identify the social necessities of the population s health, under its responsibility; to intervene in the process of health/illness of the individuals, the family and the collective; to assume the mediation role enters the diverse members of the team of the family health, among others. The nurse s activity of work also establishes a relation of interdependence with activities of other external s professionals of the health to the USFs, as the polyclinics and the hospitals. It should be noted that although a set of procedures to be under its responsibility, many times is denied autonomy to it in important decisions. Also we could verify that these professionals are submitted the precarious physical installations of the USFs, and take offense themselves at the lack of qualification concerning the proposal of the Program, of the excess of demand for attendance, of the pressures command of them and of the users, of the lack of physical and mental rest. To give account of the constant variability, one becomes necessary that the work is lead collectively, what many times does not happen. Moreover, the excess of the bureaucracy harms the quality of the attendance. Unanimous in expressing the feeling of accomplishment for the exercise of the profession, the pleasure in making what they like, they frequently reveal its impotence and frustration for being in the front line of the service provision to the health attention of the community, and not having its requirements met, by more than endeavor. The activities that place bring their difficulties and seem to extremely overload the daily work, making the job extremely stressful.Coordenação de Aperfeiçoamento de Pessoal de Nível Superior - CAPESO Programa Saúde da Família - PSF, implantado no Brasil há mais de uma década, vem se consolidando como estratégia de reorientação das práticas assistenciais de atenção à saúde. Observa-se, entretanto, que a sua implantação e desenvolvimento nos municípios brasileiros, bem como a sua compreensão por parte dos diversos atores envolvidos (gestores, profissionais e população), têm ocorrido de forma variada. Nosso estudo pretende analisar a relação entre a atividade de trabalho e a saúde das enfermeiras da ESF dos distritos sanitários I, III, IV e V, em João Pessoa PB. Temos como objetivos específicos: investigar as condições e a organização do trabalho; identificar as regulações das atividades desenvolvidas pelas enfermeiras; analisar as vivências de sofrimento e prazer e os processos de adoecimento vinculados a esse tipo de atividade. Recorremos aos aportes teóricos da ergonomia da atividade, que disponibiliza ferramentas conceituais para apreender as situações de trabalho e os modos operatórios desenvolvidos pelas enfermeiras em sua ação laboral, bem como suas estratégias de regulação das variabilidades, decorrentes da defasagem entre o trabalho prescrito e o trabalho efetivamente realizado; e da psicodinâmica do trabalho, que se debruça sobre a análise dinâmica dos processos intersubjetivos vivenciados nas situações de trabalho e suas relações com a saúde daqueles que trabalham. Do ponto de vista metodológico, foram utilizadas doze entrevistas semi-estruturadas, visando obter a máxima amplitude na descrição, compreensão e explicação do foco em estudo, e quatro observações da atividade de profissionais de diferentes distritos sanitários, que aceitaram participar voluntariamente da pesquisa. A análise dos dados produzidos foi a de conteúdo temática. Preliminarmente foi percebido que, nas Unidades de Saúde da Família, as enfermeiras desenvolvem múltiplas atividades no campo da assistência, da gerência e da educação/formação, o que amplia a sua responsabilidade. Dessa forma, são inúmeras as ações assumidas, tais como: serem capazes de identificar as necessidades sociais de saúde da população sob a sua responsabilidade; intervirem no processo saúde/doença dos indivíduos, da família e da coletividade; assumirem o papel de mediação entre os diversos membros da equipe de saúde da família, dentre outros. A atividade de trabalho da enfermeira também estabelece uma relação de interdependência com atividades de outros profissionais da saúde externos às USFs, como as policlínicas e os hospitais. Cabe ressaltar que, apesar de um conjunto de procedimentos estarem sob a sua responsabilidade, muitas vezes lhe é negada autonomia em importantes decisões. Também pudemos verificar que, essas profissionais estão submetidas a precárias instalações físicas das USFs, e ressentem-se da falta de capacitação acerca da proposta do Programa, do excesso de demanda por atendimento, das pressões das chefias e dos usuários, da falta de descanso físico e mental. Para dar conta das constantes variabilidades, faz-se necessário que o trabalho seja conduzido coletivamente, o que muitas vezes não acontece. Além disso, o excesso da burocracia prejudica a qualidade do atendimento. Unânimes em expressar o sentimento de realização pelo exercício da profissão, o prazer em fazer o que gostam, manifestam com freqüência sua impotência e frustração por estarem na linha de frente da prestação do serviço de atenção à saúde da comunidade, e não terem suas exigências correspondidas, por mais que se esforcem. As atividades que realizam trazem as suas dificuldades e parecem sobrecarregar o cotidiano de trabalho, tornando o trabalho extremamente desgastante
