Câncer infantil: a realidade vivenciada na percepção do cuidador familiar.

The increase in the survival of children with cancer has led the assistance into the context of the experience and needs of the child and the family, who experience the severity of the disease. The objective understand the experiences of family caregivers in the care with a child with cancer. Theoretically, this study is supported by the concept of the family-centered care in the perspective to understand the family carer‟s experience toward an efficient care of the family. It is a qualitative research, whose empiric material was collected throughout interviews recorded from February through April 2011, involving family carers of children with cancer, who were guests at the Casa da Criança institution. The analysis was accomplished as per the theoretical analytical referential of the Analysis of the Discourse, French line, under two discursive formations: to be family carer of a child with cancer, dealing with the unknown, and the family carer facing the child‟s cancer. The findings pointed out to the legates of devotion, abnegation and moral obligation that he imposes on himself in the socio-cultural nuances sustaining the care. In the first discursive formation, it was possible to apprehend the carer‟s conception of care, being seen as an ambiguous form of a gift of God, but also as an obligation that he imposes on himself to be the sole child‟s protector. It was also possible to apprehend the feelings experienced by the carer, in which the suffering starts as from the journey in search of the diagnosis, with failures in the organization and resolution of the services, whilst permeated by the stigma that the cancer represents, implicating in sentiments of blame and fear of the child‟s death. In the second discursive formation, it was possible to go into the family relationships involving the carer, which led us into perceiving that he resents needing to resign from home and the care with the family nucleus, but who suffocates the contact and the support to the large family, so evidencing the fragile ties in this ambience. As regards the strategies of facing, it was possible to observe in the discourses that the carers get hold of a strength and courage to struggle for the child‟s life that they themselves never thought they had, further to being supported by faith, religiousness and spirituality. The network that offers social support to the carer is formed by family care institutions. The meanings that the families attribute to their experiences widen the understanding of the concepts of the family-centered care, there allowing a way toward the reflection and the practical applicability of approach. As from the analyzed factors, we detach a few aspects that need be considered by the nurse who deals in the assistance to children with cancer and their families: the importance of an efficient dialogical relationship to allow a family member to expose his feelings and to make him feel comfortable with the care-rendering institutions; the incentive to face the cancer in family, cutting off the carer‟s overload; understanding the vulnerability of the carer who needs to be thoroughly assisted and the need to strengthen the facing strategies used by the carer.Coordenação de Aperfeiçoamento de Pessoal de Nível Superior - CAPESO aumento da sobrevida de crianças com câncer tem direcionado à assistência para o contexto da vivência e das necessidades da criança e da família que vivencia os percalços da doença. O estudo teve como objetivo compreender as vivências do cuidador familiar no cuidado à criança com câncer. Teoricamente este estudo está respaldado no conceito do cuidado centrado na família, na perspectiva de compreender a vivência do cuidador familiar para atuar eficientemente no cuidado à família. Trata-se de uma pesquisa qualitativa, que teve seu material empírico construído por meio de entrevistas gravadas no período de fevereiro e abril de 2011, realizadas com nove cuidadores familiares de crianças com câncer que se encontravam hospedados na instituição Casa da Criança. A análise foi realizada conforme o referencial teórico analítico da Análise de Discurso de linha francesa, sendo produzidas duas formações discursivas: ser cuidador familiar de uma criança com câncer: percorrendo o desconhecido e o cuidador familiar enfrentando o câncer da criança. Os achados apontaram os legados da dedicação, abnegação e obrigação moral nas matrizes socioculturais que sustentam o cuidar. Na primeira formação discursiva foi possível apreender a concepção de cuidado do cuidador, sendo vista de forma ambígua como uma dádiva de Deus, mas também como uma obrigação a que ele mesmo se impõe de ser o único protetor da criança. Foi possível também apreender os sentimentos vivenciados pelo cuidador, no qual o sofrimento inicia-se desde o itinerário percorrido na busca pelo diagnóstico, com falhas na organização e resolutividade dos serviços e é permeado pelo estigma que o câncer representa implicando nos sentimentos de culpa e temor da morte da criança. Na segunda formação discursiva foi possível adentrar nas relações familiares que envolvem o cuidador, ao percebermos que esse se ressente por necessitar abdicar do lar e dos cuidados à família nuclear, mas que silencia o contato e o apoio da família extensa evidenciando laços frágeis nessa rede. Em relação às estratégias de enfrentamento foi possível observar nos discursos que os cuidadores empoderam-se de uma força e coragem que nem mesmo eles conheciam para lutar pela vida da criança, além de estarem apoiados na fé, religiosidade e espiritualidade. A rede que oferece apoio social ao cuidador é constituída pelas instituições de cuidado à criança, bem como pelos profissionais e as outras mães que nelas se encontram e os familiares. Os significados atribuídos pelas famílias às suas experiências ampliaram a compreensão dos conceitos do cuidado centrado na família, proporcionando um caminho para a reflexão e aplicabilidade prática da abordagem. A partir dos fatores analisados destacam-se alguns aspectos que precisam ser considerados pelo enfermeiro que atua na assistência a crianças com câncer e suas famílias: a importância de uma relação dialógica eficaz que permita ao familiar expor seus sentimentos e sentir-se acolhido nas instituições prestadoras de cuidado; o incentivo ao enfrentamento do câncer em família diminuindo a sobrecarga do cuidador; a compreensão da vulnerabilidade do cuidador, que precisa ser assistido integralmente, e a necessidade de fortalecimento das estratégias de enfrentamento utilizadas pelo cuidador
