MANIFESTAÇÃO OCULAR ATÍPICA DA LEISHAMANIOSE E DIAGNÓSTICO PARASITOLÓGICO POR IMPRINT CONJUNTIVAL EM CÃO: RELATO DE CASO

The nonspecific symptoms that characterize leishmaniasis in dogs include fever, splenomegaly, ascites, severe weight loss, and anemia. There may be skin ulcerations, skin peeling, ophthalmic and periocular lesions. One-year-old male Pug was referred to the Veterinary Hospital of UFPB, Areia-PB, with history of inappetence, epistaxis and lameness about three months ago. On examination, the most obvious changes were edema and hyperemia of the medial bulbar conjunctiva of eyes, alopecia and hyperkeratosis of the lateral eyelids of both eyes, hyperkeratosis and hairy rarefaction in the perianal region. The tonometry revealed anterior uveitis. The dog had nonregenerative anemia and thrombocytopenia throughout their clinical follow-up. Discrete and temporary changes were observed in ALT and FA dosages. Serological tests for leishmaniasis as indirect immunofluorescence and ELISA revealed negative results. However, due to the persistence of clinical signs, anemia and thrombocytopenia, although symptomatic treatment, leishmaniasis was not ruled out. Therefore, it was requested molecular examination of real-time PCR of total blood, that the result was also negative. Thus, conjunctival cytology, peripheral blood cytology and conjunctival biopsy were performed to further investigate the disease. Such parasitological examinations concluded diagnosis through observation of numerous amastigotes of Leishmania spp. on macrophages. A therapeutic protocol for leishmaniasis was established for the animal, with good clinical response.A sintomatologia inespecífica que caracteriza a leishmaniose em cães compreende febre, esplenomegalia, ascite, emagrecimento acentuado e anemia. Pode haver ulcerações cutâneas, descamação da pele, lesões oculares e perioculares. Um cão da raça Pug, de um ano e oito meses de idade, foi encaminhado ao Hospital Veterinário da UFPB, Areia-PB, com histórico de inapetência, episódios de epistaxe e claudicação há aproximadamente três meses. Ao exame, as alterações mais evidentes foram o edema e a hiperemia da conjuntiva bulbar medial de ambos os olhos, alopecia e hiperceratose em canto lateral das pálpebras, bilateralmente, hiperceratose e rarefação pilosa na região perianal. A tonometria de aplanação revelou uveíte anterior. O cão apresentou anemia arregenerativa e trombocitopenia durante todo o seu acompanhamento clínico. Alterações discretas e temporárias foram observadas nas dosagens sérias de ALT e FA. Testes sorológicos como a imunofluorescência indireta e ELISA, para leishmaniose, revelaram resultados negativos. Contudo, devido à persistência de sinais clínicos, da anemia e da trombocitopenia, apesar de tratamento sintomático, a leishmaniose não foi descartada. Para tanto, foi solicitado exame molecular de PCR em tempo real de sangue total do animal, cujo resultado também foi negativo. Desta maneira, foram realizadas citologia esfoliativa da conjuntiva e do sangue periférico, além de biópsia conjuntival, para melhor investigar a doença. Tais exames parasitológicos concluíram o diagnóstico por meio da observação de inúmeras formas amastigotas de Leishmania spp. em macrófagos das amostras. Um protocolo terapêutico para leishmaniose foi estabelecido para o animal, com boa resposta clínica
