Correlation between TMJ temporal component pneumatization and morphology   : analysis by cone beam computed tomography

O conhecimento dessas condições pode fornecer valiosas informações na avaliação da articulação temporomandibular (ATM). Sendo assim, o objetivo desse estudo foi correlacionar a pneumatização do osso temporal e a morfologia da eminência articular e da fossa mandibular. Além disso, foi proposta uma classificação para a pneumatização do componente temporal da ATM. Foi utilizada uma amostra de 100 exames de tomografia computadorizada de feixe cônico (TCFC) totalizando 200 ATMs. Esses exames foram retirados do acervo de imagens digitais do biobanco da Faculdade de Odontologia de Piracicaba da Universidade Estadual de Campinas - FOP/UNICAMP. As imagens foram avaliadas qualitativa e quantitativamente por meio das reconstruções paracoronal e parassagital da ATM por dois examinadores em consenso. Todas as ATMs foram classificadas quanto à pneumatização no sentido médio-lateral em escores entre 0-1, e no sentido ântero-posterior em escores entre 0-3. A inclinação e a altura da eminência articular e a espessura do teto da fossa mandibular foram obtidas. As medidas lineares e angulares encontradas para as diferentes classificações de pneumatização foram comparadas pela Análise de Variância (ANOVA dois fatores ¿ pneumatização x lado). A pneumatização esteve presente em 83,5% dos casos no sentido médio-lateral e em 88% no sentido ântero-posterior. ANOVA indicou que o lado ou a presença da pneumatização não afetaram significativamente a morfologia da eminência articular e da fossa mandibular (p>0,05). Foi possível concluir que não existe uma relação entre a presença da pneumatização no componente temporal da ATM e sua morfologia. No entanto, os profissionais devem ter em mente a alta prevalência da pneumatização e levar isso em consideração ao realizar a avaliação da ATM. Além disso, foi proposta uma classificação para a pneumatização do componente temporal da ATM considerando a sua extensão nos sentidos ântero-posterior e médio-lateral quando analisada em imagens de TCFCAbstract: The individual variation of temporal bone morphology is significant and its degree of pneumatization is variable. Knowledge of these conditions can provide valuable information in temporomandibular joint (TMJ) evaluation. Therefore, the aim of this study was to evaluate whether a correlation exists between temporal bone pneumatization and the morphology of the articular eminence and glenoid fossa. Additionally, a classification of pneumatization of the temporal component of the TMJ is proposed for use in cone beam computed tomography (CBCT). A sample of 100 CBCT scans was used, totaling 200 TMJs. These exams were chosen from the database of digital images of Piracicaba Dental School, University of Campinas - FOP/UNICAMP. Paracoronal and parasagittal images of the TMJ were evaluated qualitatively and quantitatively by two examiners. In all TMJs, pneumatization was classified in the mid-lateral direction using a score of 0 or 1, and in the anteroposterior direction using a score ranging from 0-3. The inclination and height of the articular eminence and the thickness of the roof of the glenoid fossa were obtained. The linear and angular measurements found for the different pneumatization classifications were compared by two-way ANOVA (pneumatization x side). Pneumatization was found in the mid-lateral direction in 83.5% of the cases and in the anteroposterior direction in 88%. The side or presence of pneumatization did not significantly affect articular eminence and mandibular fossa morphology (p>0.05, ANOVA). It was concluded that the presence or extent of pneumatization of the TMJ temporal component does not affect its morphology. However, professionals should be aware of the high prevalence of pneumatization and take it into account when performing TMJ assessment.
