Cross-cultural adaptation, validation and reability of the brazilian version of Spinal Cord Independence Measure Self Reported (brSCIMSR)

During the process of rehabilitation, the independence limitation is an important aspect of evaluation and multidisciplinary treatment in people with spinal cord injury (SCI). Instruments have been developed to assess the level of independence. Among these, we highlight the Spinal Cord Independence Measure (SCIM), specifically designed to evaluate people with SCI through observation or interview. Recently, a self-report version of Spinal Cord Independence Measure - Self Reported (SCIM-SR) was developed from the SCIM III to facilitate, through its mode of application, analysis of the level of functional independence in persons with SCI. The aim of the study was cross-cultural validation of SCIM-SR and analyze their reliability. Thirteen individuals with SCI were included in the study. They were evaluated: the concurrent validity, internal consistency, test-retest reliability, the measurement error and the floor and ceiling effect. The analysis of criterion validity demonstrated a strong correlation of the brazilian version of SCIM-SR (brSCIM-SR) with total FIM (r = 0.721; P = 0.005) and motor FIM (r = 0.766; P = 0.005) as well as the Brazilian version of SCIM III (r = 0.946; p = 0.001). A high internal consistency was found (α = 0.92). Considering the total score to brSCIM-SR showed excellent test-retest reliability (ICC 0.99; p = 0.001). The measurement error (EM) was 2 points, and there was no effect floor and ceiling. The Brazilian version of the self-report SCIM-SR is a valid, reliable, low cost and easy applicability being able to quantify the level of functional independence of people with SCI. This way can be a useful tool both for research and for clinical practice in there habilitation of this population.Durante o processo de reabilitação, a limitação da independência é um um importante aspecto de avaliação e tratamento multidisciplinar em pessoas com lesão medular espinal (LME). Diversos instrumentos foram desenvolvidos para avaliar o nível de independência. Entre estes, destacamos a Spinal Cord Independence Measure (SCIM), especificamente desenvolvida para avaliar pessoas com LME através da observação ou entrevista. Recentemente, uma versão de autorrelato da Spinal Cord Independence Measure Self Reported (SCIM-SR) foi desenvolvida a partir da SCIM III para facilitar, através do seu modo de aplicação, a análise do nível de independência funcional em pessoas com LME. O objetivo do estudo foi realizar a validação transcultural da SCIM-SR e analisar a sua confiabilidade. Treze indivíduos com LME foram inclusos no estudo. Foram avaliadas: a validade de concorrente, consistência interna, a confiabilidade teste reteste, o erro de medida e o efeito piso e teto. A análise da validade concorrente demonstrou uma forte correlação da versão brasileira da SCIM-SR (brSCIM-SR) com a MIF total (r=0,721; p=0,005) e MIF motora (r=0,766; p=0,005), bem como com a versão brasileira da SCIM III (r=0,946; p=0,001). Uma alta consistência interna foi encontrada (α=0,92). Considerando o escore total a brSCIM-SR apresentou uma excelente confiabilidade teste-reteste (CCI 0,99; p=0,001). O erro de medida (EM) foi de 2 pontos, e não houve efeito piso e teto. A versão brasileira de autorrelato da SCIM-SR é um instrumento válido, confiável, de baixo custo e fácil aplicabilidade sendo capaz de quantificar o nível de independência funcional das pessoas com LME. Dessa maneira pode ser uma ferramenta útil tanto para a pesquisa, quanto para a prática clínica na reabilitação dessa população.Coordenação de Aperfeiçoamento de Pessoal de Nível Superio
