PENURUNAN TEKANAN DARAH PASIEN HIPERTENSI STAGE I RAWAT JALAN SETELAH PEMBERIAN KAPTOPRIL PAGI DAN MALAM DI RSUD KABUPATEN LOMBOK UTARA

Hypertension is often referred to as the silent killer. One of antihypertensive drugs most widely used is captopril. Based of the short duration of action of captopril, the schedule becomes important to take medication to keep the blood pressure in the morning and evening (circadian blood pressure). This study aims to determine the decrease in blood pressure in the morning and evening after administration of captopril 25 mg every 12 hours compared to the group without taking medication schedule. This study is a quasi experimental. Subjects were patients with stage I hypertension aged 18-60 years who received a single antihypertensive captopril 25 mg two times a day for 14 days. Intervention treatment group (n=43) taking medication schedule every 12 hours, while the control group (n=43) without intervention. Blood pressure measurements performed on days 0, to 8 and 15 in the morning (06:00 a.m. to 09:00 a.m.) and night (6:00 p.m. to 9:00 p.m.). The results observed were a decrease blood pressure in systolic and diastolic in the morning and evening. The decrease in blood pressure in the morning after 15 days of intervention was 17.90 ± 7.89 / 2.09 ± 5.15 mm Hg and in control was 13.37 ± 9.56 / 2.79 ± 7.66 mm Hg. Deterioration in morning systolic differ significantly between the two groups, whereas the decline in early diastole between the two groups did not differ significantly. Decrease in blood pressure at night after 15 days of intervention was 18.02 ± 8.46 / 1.86 ± 6.27 mmHg and the control was 12.72 ± 12.72 / 2.33 ± 6.11 mmHg. Deterioration in systolic and diastolic night did not differ significantly between the two groups. Administration of captopril 25 mg every 12 hours in patients with stage 1 hypertension is better to control blood pressure in morning. Overall duration of captopril affects more consistent reduction in blood pressure than taking medication schedule
