ANALISIS MUTASI GENA ALPHA DETERMINANT VIRUS HEPATITIS B PADA DONOR DARAH DENGAN INFEKSI HEPATITIS B TERSAMAR DI UNIT PELAYANAN TRANSFUSI DARAH RSUP DR. SARDJITO

Background Occult hepatitis B infection (OBI) is a great challange in blood donation. However, HBsAg is still marker of choice in blood transfusion unit which lead to unsafety blood donation. Despite having a great association with hepatocellular carcinoma and Chirrosis, OBI is still not well documented, especially in Yogyakarta. In the previous study, it is found that occult hepatitis B infection occurs because of mutation at �a� determinant region of HBsAg that may affect the antigenicity. This study was aimed to identify mutation within �a� determinant region of HBsAg in blood donors at UPTD RSUP Dr. Sardjito. Method Serum of 150 Blood donors were tested with HBsAg, anti-HBs, and anti-HBc. All samples contain HBV DNA were isolated and amplified with PCR and read using electroforesis. Sequence were performed in each of 18 samples which have HBV DNA positive. Result Of the 150 HBsAg negative samples (135 males, 15 females, age 31,2 ± 10,1 years), 21 (14,0%) have anti- HBs positive, 39 (26,0%) have anti-HBc positive, and 11 (7,3%) have both anti-HBs and anti-HBc positive. Electroforesis and sequencing showed 18(12,0%) samples (17 males, 1 female, age 43,8 ± 8,4 years p<0,05) have HBV DNA positive. From that viremic samples, 9(50,0%) had subtitutions (T131N, Q129R, Q129H, M133T) at 'a determinant region (age 43,4±8,6 years with p>0,05). Conclusion 'a' determinant region mutation were detected in blood donors at Blood Transfusion Unit of RSUP Dr. Sardjito that contribute to occult hepatitis B infection
